Covered when ALL of the following clinical criteria and documentation requirements are met:

Services are reimbursable only when the request and documentation meet the policy's clinical criteria and the order/documentation requirements (patient-specific signed order, rationale, specimen details, collector and provider identity).

Presumptive urine screening - covered situations

Assessment of individuals treated for chronic, non-cancer pain when history/signs/symptoms suggest use of non-prescribed medications or illegal substances, including prior to initiating chronic opioid therapy and for routine monitoring with random testing interval and drug selection based on history and treatment as documented in the medical record.
Testing for individuals with aberrant behavior (e.g., lost prescriptions, multiple early refill requests, opioids from multiple providers, unauthorized dose escalation, apparent intoxication) at the time of visit.
Pregnant individuals at high risk for substance use when suspicion is documented (screening answers or PDMP information).
Newborns when there is a history of maternal substance abuse or altered mental status in the birthing parent.
Organ transplant candidates with a history of substance abuse to demonstrate abstinence prior to transplant.
Individuals with suspected or diagnosed mental illness where drug testing is clinically indicated.
Individuals with attention‑deficit/hyperactivity or disruptive behavior disorders when clinically indicated.
Cancer patients on opioid pain medication when clinically indicated.
Individuals with epilepsy when drug testing is clinically indicated.
Individuals in substance use treatment: qualitative testing per documented monitoring plan—0–90 days abstinent: 1–2/week; >90 days abstinent: 1–3 per month (after baseline).

Presumptive screening - monitoring frequencies for chronic opioid therapy

Monitoring of individuals on opioid therapy to ensure adherence, inform treatment planning, or detect non‑prescribed opioids.
No adequate presumptive test exists or the presumptive test does not adequately detect the specific drug/metabolite.
To definitively identify a specific drug within a large drug family.

Definitive testing frequency limits by abstinence duration

0–30 consecutive days abstinent: random definitive testing not to exceed 1 per week.
31–90 consecutive days abstinent: random definitive testing 1–3 per month; no more than 3 definitive tests in one month.
>90 consecutive days abstinent: definitive testing 1–3 tests every three months; no more than 3 definitive tests in a 3‑month period.
