General Inflammation Testing (CRP and ESR) Reimbursement
This policy governs BCBSOK reimbursement criteria for measurement of C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) for inflammatory conditions and general inflammation testing for covered members and providers.
Reimbursement information revised to provide specific listing of inflammatory conditions for ESR, CRP or both ESR/CRP testing including frequency.
CRP is listed first due to preference for CRP over ESR.
For individuals without a diagnosed inflammatory condition, measurement of erythrocyte sedimentation rate (ESR) is not reimbursable.
Frequency guidance for Acute Hematogenous Osteomyelitis (AHO) and Giant Cell Arteritis (GCA) updated to specify timing (to confirm diagnosis; 2–3 days early therapeutic course; weekly until normalization for AHO; and to confirm diagnosis and during follow-up visits for GCA).
Table 1 introductory statement updated to indicate CRP can be conventional or high-sensitivity.
