Coverage Criteria for Biomarker Testing

Primary coverage criteria

Reimbursable testing and required clinical indications

CSF oligoclonal band analysis for MS diagnosis

For the diagnosis of multiple sclerosis (MS), cerebrospinal fluid (CSF) and serum oligoclonal band analysis may be reimbursable in any of the following situations:
ANY of the following
- Atypical clinical, laboratory, or imaging features
- Atypical clinically isolated syndrome including, but not limited to, primary progressive multiple sclerosis or relapsing-remitting course
- Belonging to a population in which MS is less common (e.g., children, older individuals)
- Insufficient clinical or imaging evidence for diagnosis

Serum AQP4-IgG and MOG-IgG testing for suspected NMOSD or MOG-EM

Serum indirect fluorescence assay or fluorescence-activated cell sorting (FACS) assay of aquaporin-4-IgG (AQP4-IgG) and MOG-IgG may be reimbursable when ALL of the following conditions are met:
The individual has monophasic or relapsing acute optic neuritis, myelitis, brainstem encephalitis, encephalitis, or any combination thereof
The individual has radiological or electrophysiological findings compatible with central nervous system (CNS) demyelination
ANY of the following
- Belongs to a higher risk population (e.g., pediatric)
- Has an abnormal MRI depicting extensive optic nerve lesion, extensive spinal cord lesion or atrophy, or large confluent T2 brain lesions

Non-reimbursable tests

In all other situations, serum biomarker tests for multiple sclerosis are not reimbursable.
ELISA, Western blot, immunohistochemistry, or any other serum assays to test for NMOSD or MOG-EM are not reimbursable.
For the diagnosis of MS, NMOSD, or MOG-EM, all other cerebrospinal fluid (CSF) biomarker tests, including AQP4-IgG or MOG-IgG, are not reimbursable.

Documentation and claims submission — provider responsibilities

Providers are responsible for submitting accurate documentation of services performed and must code claims using valid, industry-standard code sets and guidelines. Claims are subject to code-edit protocols and review; upon request providers should submit additional medical records or other supporting documentation.

Submit claims using valid code combinations from HIPAA-approved code sets (CPT, HCPCS, ICD-10-CM/PCS, NDC, DRG, etc.).
Code appropriately per industry-standard guidance (Uniform Billing Editor, AMA CPT, CPT Assistant, CMS NCCI policy manual and table edits).
Be prepared to provide additional documentation upon request for claim review related to benefit coverage, provider contracts, medical policies, or coding software logic.

Definitions and Specific Test Situations

CSF oligoclonal band analysis reimbursable situations

Reimbursable clinical indicationsCSF oligoclonal band analysis may be reimbursable for diagnosis of MS when any of the following apply: atypical clinical, laboratory, or imaging features; atypical clinically isolated syndrome (including primary progressive or relapsing-remitting course); belonging to a population in which MS is less common (e.g., children, older individuals); or insufficient clinical or imaging evidence for diagnosis.

Non-reimbursable CSF/serum biomarkersFor the diagnosis of MS, NMOSD, or MOG-EM, all other CSF biomarker tests, including AQP4-IgG or MOG-IgG, are not reimbursable.

Related serum test exclusionIn all other situations, serum biomarker tests for multiple sclerosis are not reimbursable.

AQP4-IgG and MOG-IgG testing reimbursable situations

Permitted assay typesSerum indirect fluorescence assay or fluorescence-activated cell sorting (FACS) assay for AQP4-IgG and MOG-IgG may be reimbursable when all listed clinical conditions are met.

Required clinical presentationIndividual has monophasic or relapsing acute optic neuritis, myelitis, brainstem encephalitis, encephalitis, or any combination thereof.

Required supportive findingsRadiological or electrophysiological findings compatible with central nervous system demyelination must be present.

Additional high-risk features (at least one required)Examples include pediatric age, abnormal MRI with extensive optic nerve or spinal cord lesions or large confluent T2 brain lesions, prominent papilledema/papillitis, neutrophilic CSF pleocytosis, histopathology showing pattern II MS, simultaneous bilateral acute optic neuritis, severe visual deficit or blindness, severe/frequent myelitis or brainstem encephalitis, permanent sphincter/erectile disorder after myelitis, or previous diagnosis of ADEM.

Excluded assay methodsELISA, Western blot, immunohistochemistry, or any other serum assays to test for NMOSD or MOG-EM are not reimbursable.
