Provider documentation and plan review

Providers are responsible for submitting accurate and complete documentation to support billed services and medical necessity. Before rendering services and submitting claims, verify member coverage and eligibility with the member’s plan documents (e.g., Certificate of Health Care Benefits, benefit booklet, Summary Plan Description) and any applicable provider contract. Retain and provide clinical records, test reports, and supporting documentation upon request to facilitate claim review and potential appeals.

Verify member eligibility and benefits prior to testing.
Include clinical indication, relevant history, and test-specific rationale in the medical record.
Respond to requests for additional documentation promptly to avoid delays or denials.

Claim review and adjudication

Claims are subject to review and adjudication based on benefit terms, provider contract language, medical policies, clinical payment and coding policies, and coding/software edit logic. Providers should bill using valid HIPAA-compliant codes and follow industry-standard coding guidelines. Incomplete, miscoded, or unsupported claims may be denied or downcoded.
