ALL of the following

Serologic monitoring after diagnosis
- IgA anti-TTG serology for individuals diagnosed with celiac disease who are IgA sufficient
  Monitoring intervals: first follow-up 3–6 months after diagnosis; every 6 months until normalization; then every 12–24 months
- For individuals diagnosed with celiac disease who are IgA deficient: IgG endomysial antibody, IgG deamidated gliadin peptide, or IgG TTG testing
  Monitoring intervals: first follow-up 3–6 months after diagnosis; every 6 months until normalization; then every 12–24 months
Initial diagnostic serology or diagnostic substitution
- IgA anti-TTG plus total IgA testing for individuals with signs or symptoms of celiac disease (see Note 1)
  As indicated for diagnosis
- When IgA anti-TTG is negative or weakly positive in individuals at risk for celiac disease: IgA endomysial antibody testing
  As clinically indicated
- For individuals with clinical suspicion of celiac disease who are IgA deficient: IgG endomysial, IgG deamidated gliadin peptide, or IgG TTG testing

Documentation, Claims, and Provider Requirements

Documentation Required

Documentation and claim review — Providers must submit accurate documentation; claims subject to benefit determinations and may require additional documentation on request.

Providers are responsible for submitting accurate documentation of services performed and for submitting claims using valid code combinations from HIPAA-approved code sets. Claims must be coded according to industry-standard coding guidelines, including but not limited to: Uniform Billing Editor, American Medical Association Current Procedural Terminology (CPT), CPT® Assistant, Healthcare Common Procedure Coding System (HCPCS), ICD-10-CM and PCS, National Drug Codes (NDC), Diagnosis Related Group (DRG) guidelines, Centers for Medicare & Medicaid Services (CMS) National Correct Coding Initiative (CCI) Policy Manual, and CCI table edits. Claims are subject to code edit protocols and claim review including, but not limited to, benefit coverage terms, provider contract language, medical policies, clinical payment and coding policies, and coding software logic. Upon request, providers must submit any additional documentation to support the claim. If a conflict arises between this Clinical Payment and Coding Policy and a member's plan document, the plan document governs; if a conflict arises between this policy and a provider contract, the provider contract governs. Blue Cross and Blue Shield of Oklahoma may use reasonable discretion in interpreting and applying this policy and has discretionary authority as provided under applicable plan documents.
