Coverage Criteria for Modifier 22

Modifier 22 may be considered for additional reimbursement when documentation demonstrates substantially greater work than usual. Certain uses are not appropriate.

Modifier 22 must be appended only when the provider represents the service was substantially greater than usually required and thorough documentation is included in the medical record.

Reasons that may justify Modifier 22 include

Increased time not described by a more comprehensive code
Increased intensity
Technical difficulty of the procedure not described by a more comprehensive code
Physical and mental effort that was required
Severity of the patient's condition

Documentation requirements

Thorough operative or procedure documentation specifying how the procedure was more complex than usual and quantifying how much more complex the procedure was compared to the usual must be included in the medical record.
Brief letters or statements that are not part of the medical record are insufficient.

Field avoidance — anesthesia (special exception)

Modifier 22 may be appended for field avoidance when access to the airway is limited and the anesthesia service required substantially greater work compared to the typical patient.
Documentation supporting the additional work and reason must be included in the member's anesthesia record.
The anesthesia base units must be less than 5.
ANY of the following
- Procedure is performed around the head, neck, or shoulder girdle.
- Position requires a position other than supine.

Appending Modifier 22 does not guarantee additional reimbursement; the Plan reserves the right to request supporting documentation and may deny or adjust payment based on review.
