Reimbursable use — initial diagnosis

Covered when ALL of the following are met

Saturation/3D mapping initial diagnosis criteria: Saturation prostate biopsy or comprehensive 3D mapping with biopsy involving 12‑core extended sampling may be reimbursable in the initial diagnosis of prostate cancer as a follow up to abnormal PSA results, presence of a palpable nodule on digital rectal examination, or suspicious radiologic findings.

See specimen handling Note 1: one vial per sextant, with no more than two core samples per vial.

Not reimbursable uses

Excluded uses: Prostate saturation biopsy or comprehensive 3D mapping with biopsy are not reimbursable for the diagnosis, staging and management of prostate cancer.

Saturation prostate biopsy and comprehensive 3D template mapping with biopsy are not reimbursable for the diagnosis, staging, or management of prostate cancer. This exclusion applies to claims submitted for purposes other than the initial diagnostic evaluation. Saturation prostate biopsy or comprehensive 3D mapping with biopsy may be reimbursable only for initial diagnosis of prostate cancer (e.g., follow-up to abnormal PSA, palpable nodule, or suspicious imaging). Claims submitted for uses other than initial diagnosis, staging, or management risk denial.

Background

Saturation prostate biopsy and 3D template mapping with biopsy are more extensive prostate sampling techniques that use a 12-core extended sampling approach and may be used after an abnormal PSA, a palpable nodule on digital rectal exam, or suspicious imaging to establish an initial diagnosis of prostate cancer. When performed for initial diagnostic evaluation, these techniques may be reimbursable as described in the policy; however, specimen handling requirements apply (see Note 1).

Note: Specimens should be processed as one vial per sextant with no more than two core samples per vial, per the policy's specimen handling instruction.
