Clinical diagnostic criteria: a. An individual presents with symptoms of painful sensory neuropathy; b. There is no history of a disorder known to predispose to painful neuropathy (e.g., diabetic neuropathy, toxic neuropathy, HIV neuropathy, celiac neuropathy, inherited neuropathy); c. Physical examination shows no evidence of findings consistent with large-fiber neuropathy, such as reduced or absent muscle-stretch reflexes or reduced proprioception and vibration sensation; d. Electromyography and nerve-conduction studies are normal and show no evidence of large-fiber neuropathy.

All four conditions must be satisfied for reimbursement.

Skin biopsy with measurement of epidermal nerve fiber density (ENFD) is a diagnostic test for suspected small-fiber (painful sensory) neuropathy and is reimbursable only when the documented clinical presentation meets all policy criteria. Specifically, reimbursement is allowed when the patient presents with symptoms of painful sensory neuropathy; there is no history of a disorder known to predispose to painful neuropathy (for example, diabetic, toxic, HIV-related, celiac, or inherited neuropathies); the physical examination shows no evidence of large-fiber neuropathy (such as reduced/absent deep tendon reflexes or reduced proprioception and vibration); and electromyography and nerve-conduction studies are normal and do not show large-fiber involvement. Claims for ENFD testing that do not meet these clinical criteria may be denied.
