Covered Indications and Criteria

Percent free PSA and short-interval follow-up: Percent free PSA testing, or a follow-up total PSA in 6-12 months, may be reimbursable when any of the following are met: (a) Individuals <76 years with total PSA >3 ng/ml and/or a very suspicious DRE; (b) Individuals >75 years (very healthy with little or no comorbidity) with total PSA >4 ng/ml or a very suspicious DRE; (c) Individuals thought to be at higher risk despite at least one prior negative prostate biopsy.>3 ng/ml; >4 ng/ml

Percent free PSA is not indicated as a routine first-line screening test for prostate cancer. The policy allows percent free PSA as an adjunctive or follow-up test only in specific circumstances (for example, when triaging an elevated total PSA or when follow-up testing is indicated after an initial abnormal result). Use of percent free PSA should be limited to those scenarios described under covered indications rather than being substituted for initial total PSA screening.

Routine use of percent free PSA, the free-to-total PSA ratio, and complexed PSA for population screening is explicitly not reimbursable. These tests are considered investigational or non-routine when ordered as screening tools in lieu of total PSA and are excluded from coverage for routine prostate cancer screening.

Specific Covered Indications

Screening, Diagnostic Evaluation, Follow-up and Surveillance Criteria

Covered when the following conditions are met (screening, follow-up, diagnostic evaluation, and post-treatment surveillance):

Background and Clinical Context

PSA testing (total PSA) is used for screening, diagnosis, and monitoring of prostate cancer. Screening recommendations vary by age and risk: routine annual screening may be reimbursable for men aged 50 years or older, and annual screening for men aged 40–50 years may be considered when an individual is at increased risk as determined by a physician. Screening after age 75 years is generally limited to very healthy individuals with little or no comorbidity because widespread screening in this population increases overdiagnosis. Total PSA is also reimbursable for diagnostic evaluation in individuals with signs or symptoms of prostate cancer and for follow-up, monitoring after resection or prostatectomy, monitoring response to therapy, and detection of recurrence.
