Reimbursable indications: a. Symptomatic individuals 18 years of age or older with emphysema, COPD, or asthma; b. Individuals with unexplained liver disease; c. Individuals with persistent obstruction on pulmonary function tests without identifiable risk factors (e.g., cigarette smoking, occupational exposure); d. Individuals 18 years of age or older with necrotizing panniculitis; e. Siblings of an individual with known alpha-1 antitrypsin (AAT) deficiency; f. Individuals with anti-proteinase three-positive (C-ANCA) vasculitis; g. Individuals with bronchiectasis without evident etiology.

Reflex / follow-up phenotyping

Isoelectric focusing/phenotyping may be reimbursable when ALL of the following are met:

Phenotyping after negative/discordant genotyping: Individuals who have negative genotype results for common variants or who have discordant results between AAT serum levels and proteotype, but for whom clinical suspicion of AAT deficiency remains.

Any testing performed for situations not described in the reimbursable indications is not eligible for reimbursement. Isoelectric focusing (phenotyping) is covered as a follow-up when genotype testing does not explain the clinical picture:

Phenotyping when genotype is negative or discordant: Individuals with negative genotype results for common variants OR discordant results between AAT serum levels and proteotype, when clinical suspicion of AAT deficiency persists.

Provider Actions and Billing Guidance

Prior Authorization

Plan-dependent reimbursement / prior authorization note

BCBSOK has implemented lab management reimbursement criteria; not all requirements apply to every product. Providers must review the member’s plan documents (Certificates of Benefits, benefit booklets, Summary Plan Descriptions, etc.) and any applicable provider contract to determine coverage and prior authorization requirements for the services being billed. If a conflict exists between this policy and a plan document or provider contract, the plan document or contract governs.

Plan documents may include Certificates of Health Care Benefits, benefit booklets, Summary Plan Descriptions, and other coverage documents.
BCBSOK may use reasonable discretion in interpreting and applying this policy; the provider contract or plan document prevails in case of conflict.

If a conflict arises between this Clinical Payment and Coding Policy and a plan document or provider contract, the plan document or contract will govern. Providers should be aware BCBSOK has discretionary authority to interpret and apply this policy to individual cases.

Documentation Required

Documentation and coding expectations

Providers are responsible for submitting accurate and complete documentation to support services billed. Claims must use valid HIPAA-approved code sets and follow industry-standard coding guidelines (e.g., UB, AMA CPT, HCPCS, ICD-10-CM/PCS, NDC, DRG, CMS NCCI). Upon request, providers should supply additional documentation to support medical necessity and coding.

Use appropriate CPT/HCPCS/ICD-10 codes and modifiers per coding guidelines.
Claims are subject to code edit protocols and claim review; providers may be asked to submit supporting documentation.

Denial Risk

Denial risk for non-listed indications

Testing for alpha-1 antitrypsin (AAT) deficiency is not reimbursable for indications other than those specifically listed in the policy. Providers should verify that the tested indication meets the policy’s reimbursable criteria before submitting claims to reduce risk of denial.

Reimbursable indications are limited to the situations enumerated in the policy (see policy for full list).
For situations not described in the policy, AAT testing is considered not reimbursable.

Testing for AAT deficiency for indications other than those listed in this policy is not covered and will not be reimbursed. Orders and claims for AAT quantification, phenotyping, or proteotyping that do not meet a listed reimbursable scenario should not be expected to receive payment.

Background

Alpha-1 antitrypsin (AAT) deficiency is a genetic disorder associated with early-onset emphysema/COPD and liver disease. Diagnostic evaluation commonly includes serum AAT protein quantification with reflex or concurrent AAT phenotyping (isoelectric focusing) or AAT proteotyping (Pi-typing). Phenotyping/proteotyping is particularly indicated when genotype testing is negative for common variants or when genotype and serum level results are discordant but clinical suspicion persists.
