Oligoclonal band testing (CSF and serum)

Cerebrospinal fluid (CSF) and serum oligoclonal band analysis may be reimbursable for multiple sclerosis when ANY of the following are present:

Reimbursable indications: a. Atypical clinical, laboratory, or imaging features; OR b. An atypical clinically isolated syndrome such as but not limited to, primary progressive multiple sclerosis or relapsing-remitting course; OR c. Belongs to a population in which MS is less common, such as but not limited to, children, older individuals, or non-Caucasians; OR d. Insufficient clinical or imaging evidence for diagnosis.

Serum AQP4-IgG and MOG-IgG testing for suspected NMOSD/MOG-EM

Serum AQP4-IgG and MOG-IgG testing (indirect fluorescence or FACS) may be reimbursable for suspected NMOSD/MOG-EM when ALL of the following are met:

Core clinical requirements: a. Monophasic or relapsing acute optic neuritis, myelitis, brainstem encephalitis, encephalitis, or any combination thereof; AND b. Radiological or electrophysiological findings compatible with CNS demyelination;

Items a and b required.

Additional high-risk features (at least one required): i. Belong to a higher risk population-African American, Latin American, Asian, or pediatric; OR ii. Abnormal MRI depicting extensive optic nerve lesion, extensive spinal cord lesion or atrophy, or large confluent T2 brain lesions; OR iii. Prominent papilledema/papillitis/optic disc swelling during acute optic neuritis; OR iv. Neutrophilic CSF pleocytosis; OR v. Histopathology finding primary demyelination with intralesional complement and IgG deposits or previous diagnosis of 'pattern II MS'; OR vi. Simultaneous bilateral acute optic neuritis; OR vii. Severe visual deficit or blindness in one or both eyes during or after acute optic neuritis; OR viii. Severe or frequent episodes of acute myelitis or brainstem encephalitis; OR ix. Permanent sphincter and/or erectile disorder after myelitis; OR x. Previous diagnosis of acute disseminated encephalomyelitis (ADEM).

At least one of i-x required in addition to a and b.

ELISA, Western blot, immunohistochemistry, or any other serum assay methods used to detect aquaporin-4 (AQP4-IgG) or myelin oligodendrocyte glycoprotein antibodies (MOG-IgG) for neuromyelitis optica spectrum disorder (NMOSD) or MOG-associated encephalomyelitis (MOG-EM) are not reimbursable under this policy.

All cerebrospinal fluid (CSF) biomarker tests other than those specifically listed as reimbursable in this policy — including CSF testing for AQP4-IgG or MOG-IgG — are not reimbursable for multiple sclerosis, NMOSD, or MOG-EM.

Serum biomarker tests for multiple sclerosis (MS) that are not included in the policy’s specified reimbursable indications are not reimbursable. Providers should order serum biomarker testing for MS only when the clinical scenario meets the listed coverage criteria (for example, the specified indications for oligoclonal band analysis or for serum AQP4‑IgG/MOG‑IgG by indirect fluorescence or FACS).

Covered Indications

CSF and serum oligoclonal band analysis — evaluation of possible multiple sclerosis

CSF and serum oligoclonal band analysis may be used when evaluating possible multiple sclerosis in the following situations:

Indications: Atypical features, atypical clinically isolated syndrome (CIS), membership in a population with lower MS prevalence (e.g., children, older individuals, non-Caucasians), or insufficient clinical or imaging evidence for diagnosis.

Reflects covered indication language for oligoclonal band testing.

Serum indirect fluorescence assay or FACS for AQP4-IgG and MOG-IgG — high-risk clinical presentations

Serum indirect fluorescence assay or FACS assay for AQP4-IgG and MOG-IgG may be reimbursable when the core clinical features below are present AND at least one additional high-risk feature is present:

Core clinical features (both required): a. Monophasic or relapsing acute optic neuritis, myelitis, brainstem encephalitis, encephalitis, or any combination thereof; AND b. Radiological or electrophysiological findings compatible with CNS demyelination;

Core requirements per reimbursement item 2a-b.

Additional high-risk features (one or more required): i. Higher risk population (African American, Latin American, Asian, or pediatric); OR ii. Abnormal MRI with extensive optic nerve lesion, extensive spinal cord lesion or atrophy, or large confluent T2 brain lesions; OR iii. Claims documentation and coding

Providers are responsible for submitting accurate and complete documentation supporting the medical necessity of services billed. Claims must use valid HIPAA-approved code sets and follow industry standard coding guidance (e.g., UB, AMA CPT, CPT Assistant, HCPCS, ICD-10, NDC, DRG, CMS NCCI/CCI edits). Upon request, providers must submit additional documentation to support coverage and payment.

Use appropriate CPT/HCPCS/ICD-10/NDC/DRG codes and modifiers per industry guidance.
Claims are subject to code edit protocols, claim review, and payer coding software logic.
Submit clinical records, test indications, and imaging or laboratory findings when requested to substantiate medical necessity.

Services may be denied or not reimbursed in the following situations. Ensure documentation clearly supports the medical necessity and that the service is covered under the member's product before billing.

Tests billed for MS, NMOSD, or MOG-EM that do not meet the specific clinical criteria listed in this policy may be denied.
Serum biomarker tests for multiple sclerosis are not reimbursable when the policy criteria are not met.
ELISA, Western blot, immunohistochemistry, or other serum assays for NMOSD or MOG-EM are not reimbursable.
All other CSF biomarker tests, including CSF AQP4-IgG or MOG-IgG for MS, NMOSD, or MOG-EM, are not reimbursable unless specifically indicated.
Claims with incorrect, incomplete, or nonstandard coding, or lacking requested supporting documentation, are subject to denial or delay.
