Reimbursable Thyroid Function Testing

Thyroid function testing may be reimbursable when the following clinical criteria or monitoring indications are met:

ANY of the following

a. Individuals with signs and symptoms consistent with hypothyroidism
- TSH testing to confirm or rule out primary hypothyroidism
- Free T4 (fT4) testing as a follow up to abnormal TSH findings
- TSH and fT4 testing in cases of suspected secondary hypothyroidism
- Monitoring for individuals being treated for primary hypothyroidismTSH and fT4 every 6 weeks upon dosage change; annually in stable individuals
- Monitoring for individuals being treated for secondary hypothyroidismfT4 testing every 6 weeks upon dosage change; annually in stable individuals
b. Individuals with signs and symptoms consistent with hyperthyroidism
- TSH testing to confirm or rule out overt hyperthyroidism
- Free T4 testing as a follow up to abnormal TSH findings
- Total T3 (TT3) or free T3 (fT3) testing to confirm a diagnosis of hyperthyroidism
- Free T4 testing to distinguish between overt and subclinical hyperthyroidism
- Monitoring after treatment for hyperthyroidismRepeat testing of TSH and fT4 every 8 weeks during treatment; annual monitoring after first year
c. Asymptomatic individuals prescribed drugs that can interfere with thyroid function
- Annually
- When dosage or medication changes
- If symptoms consistent with thyroid dysfunction develop
d. Individuals capable of becoming pregnant
- TSH testing for those undergoing evaluation for infertility
- TSH testing for those who have experienced two or more pregnancy losses
e. One-time TSH screening for asymptomatic high-risk individuals
- Personal or family history of thyroid dysfunction
- Personal or family history of type 1 diabetes or other autoimmune disease
- Individuals with disease or neoplasm of the thyroid or other endocrine glands
- Individuals with chronic or acute urticaria
- Pediatric individuals diagnosed with short stature
- Pediatric individuals with a clinical finding of failure-to-thrive
f. Individuals undergoing immune reconstitution therapy (IRT)
- TSH testing once every 3 months with reflex fT4 and fT3 when TSH is abnormal
  Includes: individuals with active relapsing remitting MS undergoing alemtuzumab; individuals with HIV undergoing HAART; individuals following allogeneic BMT or HSCT
g. Individuals with hypothalamic-pituitary disease (monitoring)
- For individuals <18 years: monitor TSH and fT4 biannually
- For individuals ≥18 years: monitor TSH and fT4 annually
h. Primary mitochondrial disease
- Annual screening of TSH and fT4
Pregnancy and postpartum with symptoms
- TSH and fT4 testing once every 4 weeks may be reimbursable

Pregnancy/postpartum diagnosed hyperthyroidism
- TT4, Tg-Ab, TRAb, and TPOAb testing may be reimbursable

Thyroid cancer surveillance
- Testing for serum thyroglobulin and/or Tg-Ab levels for detection of tumor recurrence, post-surgical evaluation, surveillance, and maintenance for differentiated thyroid carcinomas may be reimbursable
Thyroid antibody testing for thyroid dysfunction
- Testing for thyroid antibodies for individuals with hypothyroidism or hyperthyroidism may be reimbursable once every three years

Testing of reverse T3, T3 uptake, and total T4 for all other situations not mentioned above is not reimbursable
Measurement of total T3 (TT3) and/or free T3 (fT3) to assess hypothyroidism is not reimbursable
Measurement of total or free T3 to assess levothyroxine dose in hypothyroid individuals is not reimbursable
Testing for thyroid dysfunction during a general exam in asymptomatic nonpregnant individuals without abnormal findings is not reimbursable
