Clinical indications for thyroid function testing: TSH testing is reimbursable for individuals with signs and symptoms consistent with hypothyroidism or hyperthyroidism; free T4 (fT4) testing is reimbursable as a follow-up to abnormal TSH; TSH and fT4 testing are reimbursable in cases of suspected secondary (central) hypothyroidism; TT3 or fT3 testing reimbursable to confirm hyperthyroidism.

Monitoring on therapy: For primary hypothyroidism: monitor TSH and fT4 every 6 weeks after a dosage change and annually when stable. For secondary (central) hypothyroidism: monitor fT4 every 6 weeks after a dosage change and annually when stable. For hyperthyroidism: repeat TSH and fT4 every 8 weeks during treatment; after the first year, annual monitoring for relapse or late-onset hypothyroidism.

Asymptomatic but at-risk individuals: TSH testing reimbursable for asymptomatic individuals prescribed drugs that can interfere with thyroid function at the following intervals: annually; when dosage or medication changes; and if symptoms consistent with thyroid dysfunction develop.

One-time screening indications: One-time TSH screening reimbursable for asymptomatic individuals at high risk due to: personal or family history of thyroid dysfunction; personal or family history of type 1 diabetes or other autoimmune disease; disease or neoplasm of the thyroid or other endocrine glands; chronic or acute urticaria; pediatric short stature; pediatric failure-to-thrive; individuals undergoing evaluation for infertility; individuals with two or more pregnancy losses.

Special populations and therapies: TSH once every 3 months with reflex to fT4 and fT3 when TSH is abnormal for individuals undergoing immune reconstitution therapy. For individuals following allogeneic BMT/HSCT or individuals with primary mitochondrial disease: annual TSH and fT4 screening as specified. For pregnant or postpartum individuals with symptoms of thyroid dysfunction: TSH and fT4 once every 4 weeks; in pregnancy-related hyperthyroidism, TT4, Tg‑Ab, TRAb, and TPOAb may be reimbursable. Thyroid antibody testing for hypothyroidism or hyperthyroidism may be reimbursable once every three years; serum thyroglobulin and Tg‑Ab testing may be reimbursable for thyroid cancer surveillance.
