Clinical examination and documentation: Provider documents primary and secondary diagnoses, dates and duration, and completes seating evaluation, equipment recommendations with physician justification, and prescription

If the member’s home environment does not provide adequate access, maneuvering space, appropriate surfaces, prevailing temperature, or a physical layout that allows unimpeded and safe operation of the requested Manual Wheelchair (MWC) or Power-Operated Vehicle (POV), the device is not appropriate for home use. The provider must verify that the MWC or POV will fit through doorways and can be safely operated inside the home and document that evaluation in the medical record.

Clinical Attestation Requirements

Date you examined the patient and attested to the Letter of Medical Necessity must be provided. The clinician must answer mobility- and safety-related attestation questions, including changes in medical condition affecting mobility; current and prior modes of mobility in the home; ability to safely operate a MWC and/or POV; ability to safely transfer in/out of a POV; adequacy of trunk control to safely ride in a POV; primary locations of use; anticipated duration of need (specifically whether > 6 months); and specific activities and accessory needs.

Non-Covered Items and Conditions

Requests for equipment will be denied or considered not appropriate when the home evaluation demonstrates that doorways, layout, surfaces, obstacles, or other environmental conditions would render the MWC or POV unusable or unsafe in the home. Similarly, if the patient’s physical or mental capabilities, or lack of motivation to use the device routinely, prevent safe operation, coverage is not appropriate. Providers must document the home and patient evaluation and attest that the device can be used safely in the home to avoid denial.
