Coverage Criteria for IOP Authorization

Initial Certification

Covered when ALL of the following are met:

Initial IOP Eligibility: Patient is under the care of a physician who certifies the need for intensive outpatient services, including the need for a minimum of 9 hours per week of services as evidenced by their individualized plan of care; requires comprehensive, structured, multimodal treatment under medical supervision because of a mental disorder (including substance use disorder) which severely interferes with multiple areas of daily life; is able to cognitively and emotionally participate and tolerate the intensity of an IOP program; requires more intensive treatment than outpatient but less than PHP; does not require 24-hour per day supervision and is not an imminent danger to self or others; documentation of personal and community-based support systems is provided.>=9 hours/week

Individualized written plan of care must be established by the physician and identify structured, intensive services reasonable and necessary to treat serious psychiatric symptoms and prevent relapse or hospitalization.

Recertification / Extension

Covered when ALL of the following are met:

Recertification Criteria: A treating physician who has knowledge of the patient’s response to treatment signs the recertification and documents the patient continues to require a minimum of 9 hours per week of IOP services; describes the patient’s response to therapeutic interventions provided by the IOP; lists psychiatric symptoms (including SUD) that continue to require intensive treatment; documents treatment goals and discharge coordination planning; and confirms services are furnished while the individual is under the care of a physician.recertify >= every 60 days
