Covered when the following conditions are met:

Three-day payment window applies: services rendered on the date of admission and the three calendar days preceding the date of admission are subject to this policy.

Related vs Unrelated Services

Related diagnostic or related nondiagnostic outpatient services provided by the admitting hospital within the three-day window (including date of admission) are considered inpatient services and must be included in the inpatient reimbursement.
If outpatient diagnostic or nondiagnostic services provided within the three-day window are unrelated and clinically distinct from the reason for admission, the hospital may be reimbursed separately for those outpatient services when properly documented and billed.

Billing when related

Technical portion of non-diagnostic services (unless excluded) provided by the hospital/facility within the same health system within the three-day window are considered related and must be included on the inpatient bill unless clinically unrelated.

Billing when unrelated

When non-diagnostic outpatient services are unrelated and clinically distinct, report Condition Code 51 on the outpatient claim and provide documentation to support the unrelated status.
Unrelated preadmission non-diagnostic services are not subject to the three-day payment window and may be billed separately (technical portion to be billed by the hospital or facility that provided the service).

Physician claim modifier

Append modifier PD to physician preadmission diagnostic and admission-related nondiagnostic services reported with CPT/HCPCS codes that fall within the three-day window.
If modifier PD is appended, eligible physician claims will be reimbursed as: only the Professional Component (PC) for codes with a TC/PC split; the facility rate for codes without a TC/PC split.

Exclusions & all-inclusive payment

Certain services (for example: ambulance, chemotherapy, home health, hospice, outpatient dialysis, therapies, transplants) are excluded from inclusion on the inpatient claim when not delivered on the same day as admission; review policy exclusions and provider contract language.
Services during a member's admission that are reimbursed under an all-inclusive payment method should be billed by the facility and not by a third party; third-party billed services provided while the member is admitted are the responsibility of the facility and should be included on the inpatient claim.

Documentation and review

The Plan reserves the right to request supporting documentation and will review claims within the three-day payment window; failure to adhere to coding and billing policies may affect reimbursement.

Provider documentation, coding, and claim reporting requirements

Providers must submit accurate, appropriately coded claims and retain/produce supporting documentation on request. The Plan reserves the right to request supporting documentation and will review claims within the three-day payment window; failure to adhere to coding and billing policies may impact claims processing and reimbursement.

Submit claims using valid HIPAA-approved code sets and industry-standard coding guidelines (e.g., UB, CPT, HCPCS, ICD-10-CM/PCS, DRG, CMS/NCCI guidance).
Append modifier PD to physician preadmission diagnostic and admission-related nondiagnostic services reported with HCPCS/CPT codes that fall within the three-day payment window.
If non-diagnostic outpatient services are unrelated to the admission, report condition code 51 on the outpatient claim and include supporting documentation (per brief).
Respond to Plan requests by providing additional documentation as requested; the Plan may deny or adjust reimbursement if documentation or correct coding is not provided.
