Reimbursement guidance for prostate biopsy services:

- Prostate biopsy or comprehensive 3D mapping with biopsy involving 12-core extended sampling may be reimbursable for initial diagnosis after abnormal PSA, palpable nodule on digital rectal examination, or suspicious radiologic findings.
- When clinical suspicion of prostate cancer remains after an initial negative biopsy, follow-up biopsy (excluding prostate saturation biopsy) may be reimbursable.
- Prostate saturation biopsy or comprehensive 3D mapping with biopsy are not reimbursable for the diagnosis, staging, and management of prostate cancer.
