Laboratory tests designed to confirm the diagnosis of idiopathic environmental illness are not reimbursable in all circumstances.

Screening of blood, saliva, serum, plasma, urine, and/or stool samples for volatile solvents, organic acids, and organophosphates is not reimbursable in all circumstances.

Profiling of phthalates and parabens using blood, serum, plasma, saliva, urine, and/or stool samples is not reimbursable in all circumstances.

For asymptomatic individuals

Profiling of chlorinated pesticides (including DDE and DDT) using blood, serum, plasma, saliva, urine, and/or stool samples is not reimbursable.
Testing of blood, serum, plasma, saliva, urine, and/or stool samples for carnitine sufficiency, oxidative stress/antioxidant sufficiency, detoxification adequacy, methylation status, lipoic acid and CoQ10 sufficiency, and intestinal hyperpermeability is not reimbursable.
Testing of blood, serum, plasma, saliva, urine, and/or stool samples for vitamin, mineral, or nutritional analysis is not reimbursable.

Breath testing exclusions

The use of a breath hydrogen and/or breath methane test is not reimbursable to assess or diagnose food allergies and sensitivities.
The use of a breath hydrogen and/or breath methane test is not reimbursable to assess or diagnose idiopathic environmental intolerance.
The use of a breath hydrogen and/or breath methane test is not reimbursable to assess or diagnose carbohydrate sensitivity or intolerance.
The use of a breath hydrogen and/or breath methane test is not reimbursable to assess or diagnose digestive disorders.
The use of a breath hydrogen and/or breath methane test is not reimbursable to assess or diagnose bacterial overgrowth, including but not limited to small intestinal bacterial overgrowth (SIBO).

In asymptomatic individuals and/or during general encounters without abnormal findings, testing blood, serum, urine, cerebrospinal fluid, fingernails, hair, and/or stool samples for metals is not reimbursable.

Billing rule: Specific CPT/HCPCS codes have maximum allowed units per date of service as enumerated in the Billing and Coding section; inclusion of a code in the procedure list does not guarantee coverage or reimbursement eligibility.

Providers must submit accurate documentation of services performed and code claims using valid, industry‑standard code sets and guidelines. Claims are subject to review against benefit coverage, provider contracts, medical policies, clinical payment and coding policies, and code‑edit protocols; upon request, providers should submit any additional documentation to support the claim.

Submit claims using valid code combinations from HIPAA‑approved code sets and appropriate coding guidelines (e.g., CPT, HCPCS, ICD‑10‑CM/PCS, NCCI).
Claims are subject to claim review including benefit terms, provider contract language, medical and coding policies, and coding software logic.
If requested, promptly provide additional documentation to support the services billed.
