Reimbursement Eligibility for Discarded Drugs & Biologicals

Eligibility criteria for JW-reported discarded drugs/biologicals

Covered when ALL of the following are met:

ALL of the following

Dose administered, discarded amount, exact date and time of administration, and the reason for wastage are clearly and acceptably documented in the medical record.
The discarded amount is billed on a separate line than the administered amount with the correct CPT/HCPCS code, units, and JW modifier for all non‑inpatient places of service.
The discarded drugs or biologicals are not administered to another member/patient.
The drug or biological administered is only available in a single‑use vial or single‑use package.
The drug or biological is administered appropriately for the member's medical condition and the unused portion is discarded.
Units billed correspond with the smallest dose or vial available for purchase from the manufacturer that provides the appropriate dosage for the member.
When billing for NDC‑based reimbursement, NDC codes are included and the NDC units billed correspond to the CPT/HCPCS units billed (e.g., milliliter for liquid volume, gram for weight, units of use for vial).
JW modifier is used only to report the amount discarded and must be appended on a separate claim line for discarded amounts from single‑use vials/packages; suppliers and providers must document discarded amounts in the member's medical record.

Unit billing rule

Billing unitsBill units in multiples of the dosage specified in the full CPT/HCPCS description.

RoundingIf administered amount is not a multiple of the CPT/HCPCS code, round up to the next highest unit in the CPT/HCPCS description.

Total dose limitTotal dose (administered plus wasted) must not exceed the vial amount.

Documentation and Plan Governance

Providers must submit accurate, complete documentation and follow plan governance when billing for drugs and biologicals. If a conflict exists between this Clinical Payment and Coding Policy and any plan document or provider contract, the plan document or provider contract governs. Providers are responsible for submitting claims using valid HIPAA-approved codes and following industry-standard coding guidelines (e.g., UB, AMA CPT, HCPCS, ICD-10-CM/PCS, NDCs, CMS NCCI). Claims are subject to code edits and review; upon request providers must supply supporting documentation (including clinical records) to substantiate billed services.

Document dose administered, discarded amount, exact date/time of administration, and reason for wastage in the medical record.
Use correct CPT/HCPCS codes, correct units of service, and industry-standard code combinations.
Include NDCs when billing for drugs to obtain NDC-based reimbursement; NDC units billed must correspond to CPT/HCPCS units billed.

Billing Rule

JW modifier reporting requirement

The JW modifier must be used to report discarded amounts of drugs or biologicals. The actual dose administered must be billed with the appropriate CPT/HCPCS code and units; the discarded amount must be billed on a separate line with the JW modifier for all non-inpatient places of service. Suppliers and providers must document the amount discarded in the member's medical record. Reimbursement may be provided for the discarded portion of single-use vials/packages and the amount administered up to the vial/package label amount when documentation meets plan requirements.

Append JW on a separate claim line for discarded drugs/biologicals from single-use vials or single-use packages.
JW should only be applied to the discarded amount (not the administered amount).
Ensure the discarded amount is not administered to another patient.
Units billed should correspond to the smallest dose/vial available and to NDC quantities when applicable.

Billing Rule

JZ and supplier modifier usage

Use modifier JZ when no drug wastage occurred. For ESRD patients, append AY on the same claim line with JZ when the item/service is furnished to an ESRD patient but is not for ESRD treatment. Suppliers who prepare but do not administer drugs should follow supplier-specific modifier rules: report JW if amounts were discarded during preparation, or JZ if no amount was discarded during preparation prior to supplying the drug to the patient.

Append JZ to claims when there were no discarded drug amounts.
For ESRD: AY + JZ on a single line when applicable.
Suppliers: use JW if preparation produced discarded amounts; use JZ if no discard occurred during preparation.
