Reimbursement Criteria for Vaginitis Testing

Vaginitis testing reimbursement criteria

Covered when the specified symptom or risk-factor conditions below are met; tests and panels explicitly excluded are listed under non-reimbursable rules.

For individuals with symptoms of vaginitis, pH testing, amine testing, saline wet mount, KOH (hydrogen peroxide) wet mount and microscopic examination of vaginal fluids may be reimbursable.

For individuals with symptoms of vaginitis, direct probe DNA-based identification of Gardnerella, Trichomonas, and Candida (e.g., BD Affirm VPIII) may be reimbursable.

For individuals with clinical signs and symptoms of vaginitis but negative wet-mount findings and normal pH, vaginal cultures for Candida species for diagnosis of vulvovaginal candidiasis may be reimbursable.

For individuals with symptoms of vaginitis, measurement of sialidase activity in vaginal fluid for diagnosis of bacterial vaginosis may be reimbursable.

Symptomatic individuals — standard testsFor individuals with symptoms of vaginitis, pH testing, amine testing, saline wet mount, KOH (hydrogen peroxide) wet mount, and microscopic examination of vaginal fluids may be reimbursable (item 1).

Documentation and Claim Review Requirements

Providers are responsible for submission of accurate documentation of services performed and must code claims using valid HIPAA-approved code sets and industry-standard coding guidelines (for example: UB, AMA CPT, CPT Assistant, HCPCS, ICD-10-CM/PCS, NDC, DRG, CMS NCCI/CCI edits). Claims are subject to code edit protocols and claim review, including review against benefit coverage, provider contract language, medical policies, clinical payment and coding policies, and coding software logic. Upon request, providers should submit any additional documentation to support medical necessity and coding.

Conflict Resolution and Payer Discretion

If a conflict exists between this Clinical Payment and Coding Policy and any plan document under which a member is entitled to Covered Services, the plan document will govern. If a conflict exists between this policy and any provider contract under which a provider participates or provides Covered Services, the provider contract will govern. "Plan documents" include, but are not limited to, Certificates of Health Care Benefits, benefit booklets, Summary Plan Descriptions, and other coverage documents. Blue Cross Blue Shield of New Mexico (BCBSNM) may exercise reasonable discretion in interpreting and applying this policy to services in particular cases; BCBSNM has full and final discretionary authority for interpretation and application to the extent provided under any applicable plan documents.

NAAT / PCR-based assays — definition and policy context

DefinitionNucleic Acid Amplification Test (NAAT) / Polymerase Chain Reaction (PCR) — molecular assays that detect nucleic acid targets for specific pathogens involved in vaginitis (e.g., Trichomonas vaginalis, BV-associated targets).
