- Reimbursable for pregnant individuals
ANY of the following
- Reimbursable for asymptomatic individuals undergoing urological interventions that breach the mucosa when performed prior to the procedure
ANY of the following
- Reimbursable for individuals exhibiting at least one sign or symptom of possible UTI or bacteriuria (see Note 1 for examples of signs/symptoms and population-specific lists)
ANY of the following
- Reimbursable to assess pyelonephritis

ALL of the following

ANY of the following
- Not reimbursable for all other instances of asymptomatic urinary tract infection or asymptomatic bacteriuria not specifically listed as reimbursable
ANY of the following
- Not reimbursable for follow-up urine culture testing for an uncomplicated urinary tract infection when there is evidence of clinical resolution

- Not reimbursable as part of initial screening for asymptomatic prostatitis
- Not reimbursable as part of assessment or prognosis of prostate biopsy

Note 1: Signs and symptoms of UTI/bacteriuria include bladder infection symptoms (pain or burning with urination, frequent urination, urgency despite empty bladder, bloody urine, suprapubic pressure/cramping); kidney infection symptoms (fever, chills, lower back/flank pain, nausea/vomiting); pediatric symptoms (infants/toddlers: fever, abdominal pain/fullness, foul-smelling urine, weight loss, irritability, vomiting, poor feeding, fatigue, jaundice, diarrhea; older children: daytime accidents/bedwetting, dysuria, lower abdominal/back/side pain, malodorous/cloudy/hematuric urine, nausea/vomiting, fever, fatigue, urinary urgency/ frequency with small volumes); and older/frail or catheterized individuals: worsening incontinence, new rigors (shivering/shaking), and/or changes in behavior such as agitation or confusion.

Claims are subject to claim review, applicable plan documents, provider contract language, medical policies, clinical payment and coding policies, and code edit protocols, and may be denied if documentation or coding does not support the billed service.
