PSA testing coverage criteria

Covered when the following criteria are met:

PSA testing reimbursement follows the specified screening, diagnostic, follow-up, and monitoring criteria below.

Screening — total PSA (age and risk-based)

Average-risk individuals aged 45-75 years: screening with total PSA may be reimbursable.
Higher-risk individuals aged 40-75 years: annual screening with total PSA may be reimbursable for individuals with African ancestry, germline mutations that increase prostate cancer risk, or a suspicious family history.
Individuals over 75 years: screening with total PSA may be reimbursable only for very healthy individuals with little or no comorbidities; testing after 75 should be selective per NCCN guidance.

Recommended repeat screening intervals (individuals <75 years)

If prior total PSA < 1 ng/mL and DRE normal (if done): repeat screening at 2-4 year intervals.
If prior total PSA 1-3 ng/mL and DRE normal (if done): repeat screening at 1-2 year intervals.
If prior total PSA > 3 ng/mL or very suspicious DRE: one of the following may be reimbursable: follow-up in 6-12 months with total PSA or DRE; TRUS-guided biopsy; percent free PSA.

Repeat screening (individuals >75 years)

If total PSA < 4 ng/mL and DRE normal (if done) and no other indications for biopsy: repeat screening in select (very healthy) patients.
If total PSA > 4 ng/mL or very suspicious DRE in select very healthy individuals: one of the following may be reimbursable: follow-up in 6-12 months with total PSA or DRE; TRUS-guided biopsy; percent free PSA.

Diagnostic, monitoring, and follow-up uses for total PSA: Total PSA testing may be reimbursable for initial diagnosis in individuals with signs or symptoms of prostate cancer, for follow-up of individuals with a current or prior diagnosis of prostate cancer, for ongoing monitoring after tumor resection or prostatectomy, for monitoring response to therapy, and for detecting disease recurrence.

Percent free PSA restrictions

Percent free PSA is not reimbursable as a first-line screening test for prostate cancer.
Routine prostate cancer screening using percent free PSA, free-to-total PSA ratio, and complexed PSA tests is not reimbursable.
Follow-up testing with percent free PSA may be reimbursable in patients thought to be at higher risk despite at least one prior negative prostate biopsy.
