Covered when the following criteria are met:
Screening — total PSA (age and risk-based)
- Average-risk individuals aged 45-75 years: screening with total PSA may be reimbursable.
- Higher-risk individuals aged 40-75 years: annual screening with total PSA may be reimbursable for individuals with African ancestry, germline mutations that increase prostate cancer risk, or a suspicious family history.
- Individuals over 75 years: screening with total PSA may be reimbursable only for very healthy individuals with little or no comorbidities; testing after 75 should be selective per NCCN guidance.
