Authorization and Level Assignment Logic

Authorization and continued stay decisions are made based on MCG Intensity of Care Guidelines and submitted clinical documentation; Physician Reviewer may deny or modify requested level if documentation does not meet MCG criteria.

Preauthorization requests for NICU level of care must include clinical documentation that satisfies the corresponding MCG Intensity of Care criteria for the requested level.
MCG Care Guidelines (specified LOC references) are used to determine appropriate level and length of stay; diagnosis, procedure, comorbid conditions, and age are considered.
If submitted documentation does not meet criteria, case may be referred to a Physician Reviewer and the reviewer may deny the requested level; provider will be given opportunity to discuss with the Physician Reviewer.
If preauthorization level differs from what would be authorized based on documentation/MCG, preauthorization can be denied and a new request must be submitted for the appropriate level of care.
Inpatient claims are reviewed against approved authorizations; if claim does not align with authorization, BCBSNM may request complete medical records and itemized bills to support billed services.

What Providers Must Do

Providers submitting requests for NICU level of care or submitting charges with NICU revenue codes must provide documentation establishing that the criteria for the requested level/revenue code are satisfied. Authorization and continued-stay decisions are reviewed using MCG Care Guidelines and may be customized to reflect medical policy.

Documentation Required

Maintain and provide clinical records on request

Providers are responsible for submitting accurate clinical documentation of services performed and must provide additional records upon request. If claims do not align with preauthorizations or are selected for review, BCBSNM may request complete medical records and itemized bills to support billed services.

Cases with documentation that do not meet MCG intensity-of-care criteria may be referred to a Physician Reviewer who can deny the requested NICU level of care; the attending provider will be given an opportunity to discuss the plan of treatment and must submit a new preauthorization request if the level is denied.

It applies to providers submitting preauthorization requests and inpatient claims for NICU services under BCBSNM plans, and describes the mapping of Levels 1–4 to unique revenue codes (J0171–J0174).

Authorization and continued stay decisions are made using the externally published MCG Care Guidelines (25th Edition, 2021) to determine appropriate level and length of stay; requests for NICU level of care must be supported with clinical documentation demonstrating that the MCG intensity-of-care criteria for the requested level are met. A case may be referred to a Physician Reviewer if submitted documentation does not meet established criteria, and the reviewer may deny or modify the requested level; providers have an opportunity to discuss the plan of treatment with the Physician Reviewer and must submit a new preauthorization request if a level is denied.

Inpatient claims are reviewed against approved authorizations; if billed services or revenue codes do not align with the approved authorization, BCBSNM may request complete medical records and itemized bills to substantiate the claim. Providers are responsible for submitting accurate documentation and coding in accordance with HIPAA‑approved code sets and standard coding guidelines, and claims remain subject to BCBSNM claim review and code‑edit protocols.

Where there is any conflict between this Clinical Payment and Coding Policy and a member's plan document or a provider contract, the plan document or provider contract governs. BCBSNM retains discretion in interpreting and applying this policy to specific cases.
