Reimbursement rules for biomarker testing:

ANY of the following

ANY of the following
- Atypical clinical, laboratory, or imaging features (e.g., presentations not typical for MS).
- Atypical clinically isolated syndrome, including but not limited to primary progressive MS or relapsing-remitting course.
- Member belongs to a population in which MS is less common (for example, children or older individuals).
- Insufficient clinical or imaging evidence to establish a diagnosis of MS.

ALL of the following

Testing method: indirect fluorescence assay or fluorescence-activated cell sorting (FACS).
Clinical presentation includes monophasic or relapsing acute optic neuritis, myelitis, brainstem encephalitis, encephalitis, or any combination thereof.
Radiological or electrophysiological findings compatible with central nervous system demyelination.
AND at least one of
- Belongs to a higher-risk population (e.g., pediatric).
- Abnormal MRI with extensive optic nerve lesion, extensive spinal cord lesion or atrophy, or large confluent T2 brain lesions.

Other serum tests not reimbursable: All other serum biomarker tests for multiple sclerosis are not reimbursable.

Non-reimbursable assay methods for NMOSD/MOG-EM: ELISA, Western blot, immunohistochemistry, or any other serum assays to test for NMOSD or MOG-EM are not reimbursable.

Other CSF biomarkers not reimbursable: For diagnosis of MS, NMOSD, or MOG-EM, all other CSF biomarker tests, including CSF AQP4-IgG or MOG-IgG, are not reimbursable.

Documentation, Claim Submission, and Review

Documentation Required

Documentation and claim review — provider must submit accurate documentation

Providers must submit accurate and complete documentation and bill using valid HIPAA-approved code sets. Claims are subject to code-edit protocols and review against plan documents, provider contracts, medical policies, clinical payment and coding policies, and coding-software logic. Upon request, providers should furnish additional documentation to support the claim.

Use industry-standard coding guidelines (e.g., UB, AMA CPT, CPT Assistant, HCPCS, ICD-10-CM/PCS, NDC, DRG, CMS NCCI and policy manuals).
Claims may be denied or adjusted if coding is incorrect, incomplete, or inconsistent with submitted documentation or plan/provider contract terms.
Submit claims with appropriate code combinations and follow any plan-specific benefit or contract requirements.

Test-specific Definitions

CSF and serum oligoclonal band analysis

Primary reimbursable scenarioCSF and serum oligoclonal band analysis is reimbursable for MS diagnosis when one or more of the following apply: atypical clinical/laboratory/imaging features; atypical clinically isolated syndrome; membership in populations less likely to have MS (e.g., children, older individuals); or insufficient clinical or imaging evidence.

Scope limitationFor diagnosis of MS, NMOSD, or MOG-EM, other CSF biomarker tests (including CSF AQP4-IgG or MOG-IgG) are explicitly not reimbursable.

Documentation implicationProviders should ensure documentation demonstrates the atypical feature(s) or insufficiency of evidence that justify ordering oligoclonal band analysis.

Serum AQP4-IgG and MOG-IgG testing

Reimbursable assays and indicationSerum AQP4-IgG and MOG-IgG testing may be reimbursable when performed by indirect fluorescence assay or fluorescence-activated cell sorting (FACS) and all clinical and radiologic criteria are met.
