Reimbursement criteria and restrictions: Covered when ALL of the following are met:

Screening and diagnostic testing (ANA/ENA)

For individuals with signs or symptoms of an autoimmune disease: ANA screening reimbursable once during initial workup.
Up to two additional ANA tests per lifetime if new or more severe signs or symptoms of an autoimmune disease develop.
For individuals with an abnormal, raised ANA titer with clinical correlation to an autoimmune disorder: ENA panel testing of specific autoantibodies may be reimbursable.
For individuals with an initial positive ANA test and a diagnosis of a systemic autoimmune rheumatic disease: dsDNA testing reimbursable up to four times per year.
For individuals with a negative or low-positive ANA test, certain condition-specific antibody testing may be reimbursable when clinically indicated (examples: anti-Jo-1 for a unique myositis subset; anti-SSA in the setting of lupus or Sjögren's syndrome).

Rheumatoid arthritis (RA) testing

For individuals with painful and swollen joints suggestive of RA: testing for rheumatoid factor (RF) and/or anti-CCP antibodies reimbursable once during initial workup.
If initial RF/anti-CCP testing did not result in a diagnosis of RA, up to two additional RF/anti-CCP tests per lifetime if symptoms persist or additional RA symptoms develop.

Not reimbursable / Restrictions

Monitoring of disease with ANA testing or ANA titers is not reimbursable.
For individuals without symptoms suggestive of an autoimmune disorder, ANA and/or ENA testing is not reimbursable.
Testing of specific antibodies in the absence of a positive ANA test (except where condition-specific testing is noted above) is not reimbursable.
For asymptomatic individuals, testing of ANA and/or ENA during a wellness visit or general exam without abnormal findings is not reimbursable.
Testing for serum biomarker panels not discussed above for the diagnosis of RA (e.g., Seronegative Rheumatoid Arthritis Profile), alone or in a panel, is not reimbursable.
Management of RA using serum biomarker panel testing (e.g., Vectra DA score, PrismRA) is not reimbursable.

Operational frequency notes

Providers are responsible for submission of accurate documentation and supporting records to substantiate medical necessity and correct coding. Claims are subject to code edit protocols and review against benefit coverage, provider contract terms, clinical payment and coding policies, and claims-processing logic. Upon request, providers must furnish clinical records, test reports, and other documentation to support the claim. Providers should submit claims using valid HIPAA-approved code sets and follow industry coding guidance (eg, UB, CPT, HCPCS, ICD-10).

Reimbursement frequency and non-reimbursable tests: Biomarker testing for rheumatoid arthritis and related serum panels is subject to frequency limits and exclusions. For rheumatoid factor and anti-cyclic citrullinated peptide (anti-CCP) antibody testing, reimbursement is restricted to once per lifetime for individuals with stable symptoms; repeat testing is reimbursable only if there is a significant change in clinical status. Specific panels and tests are not reimbursable, including tests and panels not discussed above when ordered alone (for diagnosis of RA), and certain proprietary disease activity or profile tests. Codes added as not reimbursable or updated in the policy include 0446U, 0447U, 0521U, and previously added codes 86200, 86340, 86341. Examples of non-reimbursable tests added or clarified: aisle® DX Disease Activity Index, Early Sjögren's Syndrome Profile, and serum biomarker panels such as Seronegative Rheumatoid Arthritis Profile. Providers should not bill for non-reimbursable panels or for repeat routine testing outside the allowed lifetime/repeat criteria.
