Submit accurate, complete documentation for all pathology services performed. For prostate biopsy specimens, follow specimen handling guidance (one vial per sextant, with no more than two core samples per vial) and include supporting clinical information (reason for biopsy, previous biopsy results, PSA and DRE findings, imaging reports when applicable). Maintain records to support medical necessity and be prepared to provide additional documentation upon request.
