Covered indications when ALL apply

Measurement of CRP and/or ESR may be reimbursable when ALL of the following apply:

General reimbursement conditions: 1) Measurement of C-reactive protein (CRP) and/or erythrocyte sedimentation rate (ESR) aligns with a diagnosed or suspected inflammatory condition listed in Table 1; 2) For individuals without a diagnosed inflammatory condition, measurement of ESR is not reimbursable; 3) Measurement of CRP and/or ESR during a general examination without abnormal findings is not reimbursable.

Conversely, the policy explicitly excludes reimbursement for measurement of ESR in individuals without a diagnosed inflammatory condition and for measurement of CRP and/or ESR performed during a general examination with no abnormal findings.

The policy reimbursement table was updated to remove Hypereosinophilic Syndrome as a listed covered indication; it is no longer included among the conditions in Table 1 that justify reimbursement for CRP and/or ESR testing.

Measurement of ESR for individuals who do not have a diagnosed inflammatory condition is considered not reimbursable and is identified in the policy as not medically necessary for that indication.

Consistent with the updated reimbursement guidance, the policy designates measurement of ESR for individuals without a diagnosed inflammatory condition as not reimbursable. This operational change is explicitly noted in the policy update history and the coverage statements.
