Reimbursable and Exclusion Criteria

Reimbursable Testing Criteria

Testing may be reimbursable when clinical and laboratory criteria are met:

ALL of the following

Clinical suspicion of autoimmune disease is high based on signs, symptoms, and other factors
General requirement for ANA testing reimbursement

ENA panel testing (specific autoantibodies)

ENA panel testing may be reimbursable when ALL of the following are met:

ALL of the following

Patient has abnormal or raised ANA antibody titer or abnormal immunological findings in serum
Requires clinical correlation with appropriate autoimmune disorder
ENA panel targets such as nRNP, SS-A, SS-B, RNP, Scl-70, or Jo-1
Testing of these specific autoantibodies reimbursable in context above

Anti-dsDNA testing frequency

Anti-double-stranded DNA (dsDNA) testing reimbursable under specified conditions:

Initial positive ANA test and clinical correlation

Baseline requirement for subsequent dsDNA testing

Subsequent dsDNA testing limited to up to four (4) times per year<= 4 tests per year

Specific antibodies when ANA negative or low-positive

Testing of specific antibodies with negative or low-positive ANA reimbursable ONLY in the following situations (ANY):

ONE of

Testing for Anti-Jo-1 in unique clinical subset of myositis
Testing for Anti-SSA (SS-A) in the setting of lupus or Sjogren's syndrome

Not reimbursable (explicit exclusions)

Testing is not reimbursable in the following situations (ANY):

Monitoring of disease with ANA testing or ANA titers

ANA and/or ENA testing for individuals with nonspecific symptoms (including fatigue and musculoskeletal pain) if not present with other symptoms suggestive of SLE

ANA and/or ENA testing during wellness visits or general encounters without abnormal findings

Testing of specific antibodies in the absence of a positive ANA test in all other situations (i.e., outside the two listed exceptions)

Use of cell-bound complement activation products (e.g., AVISE Lupus) for diagnosis of systemic lupus erythematosus (SLE)

Billing Rule

Coding and claim submission standards

Submit claims using valid HIPAA‑approved code sets and code appropriately per industry standards. Claims are subject to code edit protocols and claim review and may be reviewed against plan documents, provider contracts, medical policies, and coding logic. Provide additional documentation upon request to support the claim.

Denial Risk

Denial risk for monitoring or inappropriate indications

Claims for ANA/ENA testing may be denied when performed for monitoring disease, for nonspecific symptoms without other findings suggestive of SLE, during wellness or general encounters without abnormal findings, or when proprietary biomarker/panel tests (e.g., AVISE/Avise CTD) are used—these applications are listed as not reimbursable.

Background and Definitions

BCBSNM has implemented certain lab management reimbursement criteria for ANA, ENA, dsDNA, and proprietary biomarker panels. The policy notes that not all requirements apply to each product and providers are urged to review Plan documents for eligible coverage for services rendered. Plan documents (for example Certificates of Health Care Benefits, benefit booklets, Summary Plan Descriptions) and applicable provider contracts govern if there is any conflict between this policy and those documents; BCBSNM may use reasonable discretion in interpreting and applying the policy. The policy effective date is January 1, 2023 with Plan CMO approval on July 27, 2022.
