Documentation must meet minimum completeness and content elements to support billed services for episodes of care; failure to provide adequate documentation may result in recoupment or denial.

Documentation must be legible to someone other than the writer; member identifying information must be included on each page.

Each record must state the service(s) rendered and support the services billed and the level of care provided on each unique date; time must be documented when applicable.

Documentation must be complete and dated; timely entry of information into a medical record is expected to be completed at the time of service or shortly thereafter and should not exceed 30 days.

Signature date within 30 days of the date of service; include signature time for hospital services when applicable.

Each entry must include author identification of the physician, other qualified health care provider, or supervising provider via handwritten signature, unique electronic identifier, or initials and rendering provider credentials as applicable.
