The policy excludes a range of diagnostic procedures from coverage for evaluation of temporomandibular joint dysfunction. Not covered diagnostic tests include electromyography (including surface EMG), kinesiography, thermography, neuromuscular junction testing, somatosensory testing, transcranial or lateral skull X‑rays, sonogram (ultrasonic Doppler auscultation), intra‑oral tracing or gothic arch tracing, muscle testing, standard dental radiographic procedures, range of motion measurements, and computerized mandibular scan.

The policy specifies certain nonsurgical, non‑operative modalities that are excluded from coverage for TMJ dysfunction treatment. Not covered therapies include electrogalvanic stimulation, iontophoresis, and biofeedback.

Prior authorization or a coverage determination may be required for items or services of questionable coverage — contact Medicare Plus Blue to confirm requirements and obtain any necessary approvals prior to delivery.

Providers may not have members sign an Advance Beneficiary Notice of Noncoverage to accept financial responsibility for noncovered items.
If a provider issues a noncovered item to a member without first obtaining a coverage determination, the member must be held harmless for all charges except for any applicable Medicare Plus Blue cost share.

Bill on the CMS 1500 (02/12), UB-04 or the 837 electronic equivalent and send electronic or paper claims to your local Blue Cross plan per Blue Cross billing instructions. Retain and provide supporting documentation (medical records, operative reports, imaging, prior authorization/coverage determination letters) upon request to substantiate medical necessity.

Denial Risk

Financial responsibility if no coverage determination

If a provider issues a noncovered item to a member without first obtaining a coverage determination from Medicare Plus Blue, the member must be held harmless for all charges except any applicable Medicare Plus Blue cost share. Providers may only collect the appropriate Medicare Plus Blue cost sharing amounts from the member. If the member elects to receive a noncovered service after a confirmed coverage determination that the item is noncovered, the member is responsible for the entire charge associated with that noncovered service (unless plan rules specify otherwise).

Providers may not have members sign an Advance Beneficiary Notice of Noncoverage to accept financial responsibility for noncovered items.

Background

Temporomandibular joint (TMJ) dysfunction is a disorder of the jaw joint and its supporting structures in which the condyle, glenoid fossa or articular meniscus become damaged or misaligned, often producing meniscal compression or tearing, progressive joint deterioration, pain, limited mouth opening and compensatory masticatory muscle fatigue. Common causes include congenital or developmental anomalies, trauma, internal derangement, ankylosis, arthritic or neoplastic disease; typical clinical manifestations include jaw clicking or popping, trismus, facial or head pain, displaced disc pain, tinnitus and bruxism.
