Medically Necessary Criteria (CAS/TCAR)

Covered when EITHER category A OR category B criteria are met and no angiographically visible intraluminal thrombus is present and the procedure can be safely performed:

A.1: Congestive heart failure (New York Heart Association Class III/IV) or left ventricular ejection fraction < 30%
A.2: Open heart surgery needed within the next 6 weeks
A.3: Recent myocardial infarction (less than 4 weeks)
A.4: Severe chronic obstructive pulmonary disease
A.5: Unstable angina (Canadian Cardiovascular Society Class III/IV)
A.6: Inability to move the neck to a suitable position for surgery

If extracranial carotid artery angioplasty is performed without stent placement, the policy requires that the same medically necessary criteria for carotid artery stent placement (CAS) or transcarotid arterial revascularization (TCAR) are met. In addition, any stent delivery system that is utilized must include embolic protection.

CAS and TCAR are considered not medically necessary when there is an angiographically visible intraluminal thrombus.
