For the services described in this policy, outpatient services are not a covered service for the following product types: Commercial Managed Care (HMO and POS), Commercial PPO and Indemnity, Medicare HMO BlueSM, and Medicare PPO BlueSM members. Providers should note that precertification/preauthorization is required if the procedure is performed inpatient for all products; separate prior authorization rules may apply for outpatient settings for certain products, but outpatient coverage is not available for the products listed above.

Treatment of peripheral arterial disease (PAD), including critical limb ischemia (CLI), using injection or infusion of stem cells — whether autologous (freshly harvested or expanded in vitro, or mobilized from peripheral blood) or allogeneic — is considered investigational. The evidence is insufficient to demonstrate an improvement in net health outcome for endpoints such as overall survival, amputation-free survival, amputation rates, symptoms, or functional status; therefore these therapies are not covered except in the context of research.

Background

Peripheral arterial disease (PAD) is an atherosclerotic syndrome associated with significant morbidity and mortality. Critical limb ischemia (CLI) represents the end stage of lower-extremity PAD, in which severe obstruction of blood flow produces ischemic rest pain, ulcers, and a substantial risk of limb loss. Stem cell approaches (autologous and allogeneic) have been investigated with varied cell sources, preparation methods, routes of administration, and outcome measures. Published trials to date are primarily phase 2 studies and small randomized trials; a meta-analysis of the lowest-bias trials has not shown consistent benefit for key outcomes such as overall survival or amputation-free survival. Larger, well-designed randomized controlled trials with longer follow-up are needed to determine whether these therapies improve clinically meaningful outcomes.
