Coverage Criteria for Sacral Nerve Neuromodulation

Urinary incontinence and nonobstructive retention — Trial stimulation (Criteria A)

A trial period of sacral nerve neuromodulation may be considered MEDICALLY NECESSARY when ALL of the following are met:

Urinary Criteria A: 1. Diagnosis of at least one of the following: (a) urge incontinence; (b) urgency-frequency syndrome; (c) non-obstructive urinary retention; (d) overactive bladder.

Conservative therapy and candidacy: 2. Documented failure or intolerance to at least 2 conventional conservative therapies (e.g., behavioral training such as bladder training, prompted voiding, or pelvic muscle exercise training; pharmacologic treatment for a sufficient duration to assess efficacy; and/or surgical corrective therapy).failure of at least 2 conservative therapies

Surgical candidacy and exclusions: 2.a The individual is an appropriate surgical candidate. 2.b Incontinence is not related to a neurologic condition.

Urinary incontinence and nonobstructive retention — Permanent implantation (Criteria B)

Permanent implantation may be considered MEDICALLY NECESSARY when ALL of the following are met:

Urinary Criteria B: 1. All criteria in A (1-2) are met.

Trial response: 2. A trial stimulation period demonstrates at least 50% improvement in symptoms over a period of at least 48 hours.>=50% improvement; >=48 hours trial

Trial may be percutaneous nerve stimulation or temporarily implanted lead prior to permanent implantation.

Fecal incontinence — Trial stimulation (Criteria A)

A trial period of sacral nerve neuromodulation may be MEDICALLY NECESSARY when ALL of the following are met:

Fecal Criteria A: 1. Diagnosis of chronic fecal incontinence with >2 incontinent episodes per week for >6 months, or for >12 months after vaginal childbirth.

Conservative therapy: 2. Documented failure or intolerance to conventional conservative therapy (e.g., dietary modification, bulking agents, pharmacologic treatment) for a sufficient duration to assess efficacy.failure of conservative therapy

Surgical candidacy and exclusions: 3. The individual is an appropriate surgical candidate. 4. Condition is not related to an anorectal malformation, large external sphincter defect (>60 degrees), visible sequelae of pelvic radiation, active anal abscesses/fistulae, or chronic inflammatory bowel disease.

Neurologic and recent surgery exclusions:

Fecal incontinence — Permanent implantation (Criteria B)

Permanent implantation may be MEDICALLY NECESSARY when ALL of the following are met:

Fecal Criteria B: 1. All criteria in A (1-6) are met.

Trial response: 2. A trial stimulation period demonstrates at least 50% improvement in symptoms over a period of at least 48 hours.>=50% improvement; >=48 hours trial

Indication-specific coverage criteria

Covered when ALL of the following are met (for urinary and fecal incontinence):

Urinary incontinence: Selected patients with urinary incontinence (urge incontinence, urgency-frequency syndrome, nonobstructive urinary retention, overactive bladder) who have failed conservative treatment and receive sacral nerve neuromodulation.failure of conservative therapy

Evidence from randomized controlled trials and long-term series demonstrates symptom reduction in selected patients.

Fecal incontinence: Selected patients with fecal incontinence who have failed conservative treatment and receive sacral nerve neuromodulation.failure of conservative therapy

Randomized trials and systematic reviews demonstrate improvements in selected patients.

Constipation: Patients with constipation who have failed conservative treatment.

Sacral nerve neuromodulation is considered investigational for other urinary or voiding applications beyond the indications with demonstrated benefit. Specifically, the policy states that use for conditions such as stress incontinence or urge incontinence due to a neurologic condition (for example, detrusor hyperreflexia, multiple sclerosis, spinal cord injury, or other chronic voiding dysfunction) is investigational and not supported as medically necessary.

Use of sacral nerve neuromodulation for constipation and chronic pelvic pain is not supported by sufficient evidence. The available trials for constipation have not consistently shown improvements in outcomes, and evidence for chronic pelvic pain is limited to systematic reviews of case series; therefore these indications lack sufficient evidence to establish a net health benefit.

The policy labels sacral nerve neuromodulation as investigational (insufficient evidence) for chronic constipation and chronic pelvic pain. Trials for constipation have produced inconsistent results, and the data for chronic pelvic pain are limited and derived mainly from case series, so additional higher-quality studies are needed to determine clinical benefit.

Sacral nerve neuromodulation for constipation and for chronic pelvic pain is specifically stated to have insufficient evidence to determine net health benefit and therefore is not established as medically necessary for these indications.

Background

Sacral nerve neuromodulation is an implantable neuromodulation therapy that delivers electrical stimulation to sacral nerve roots (commonly the S3 nerve) via leads connected to a pulse generator. It is used to modulate bladder, bowel, and pelvic function in patients who have failed conservative therapies. The policy documents randomized trials and long-term series supporting symptom reduction for selected patients with urge incontinence, urgency-frequency syndrome, nonobstructive urinary retention, overactive bladder, and fecal incontinence. Patients typically undergo a temporary trial stimulation period (percutaneous or staged lead) to assess symptom response prior to permanent implantation.
