Prostatic Urethral Lift (PUL) — Medically Necessary

Covered when ALL of the following are met

PUL medical necessity: The individual has persistent or progressive lower urinary tract symptoms (LUTS) despite medical therapy (α1-adrenergic antagonists maximally titrated, 5α-reductase inhibitors, or combination medication therapy maximally titrated) over a trial period of no less than 6 months OR the individual is unable to tolerate medical therapy; AND prostate gland volume is ≤80 mL; AND prostate anatomy demonstrates normal bladder neck without an obstructive or protruding median lobe; AND individual does not have urinary retention related to conditions other than benign prostatic hyperplasia, urinary tract infection, or recent prostatitis (within past year); AND individual has had appropriate testing to exclude diagnosis of prostate cancer; AND individual does not have a known allergy to nickel, titanium or stainless steel.prostate volume ≤80 mL; medical therapy ≥6 months

Transurethral Water Vapor Thermal Therapy (Rezum) — Medically Necessary

Covered when ALL of the following are met

Rezum medical necessity: The individual is age 50 or older; has persistent or progressive lower urinary tract symptoms despite medical therapy (α1-adrenergic antagonists maximally titrated, 5α-reductase inhibitors, or combination medication therapy maximally titrated) over a trial period of no less than 6 months OR the individual is not a suitable candidate for anesthesia or is unable to tolerate medical therapy; AND IPSS score ≥12; AND prostate gland volume is ≤80 mL; AND individual does not have an active urinary tract infection or prostatitis within the past year; AND if urinary retention is present, candidate only if cannot tolerate anesthesia or is a suboptimal candidate for anesthesia; AND individual has had appropriate testing to exclude diagnosis of prostate cancer.age ≥50; IPSS ≥12; prostate volume ≤80 mL

Waterjet Tissue Ablation (Aquablation) — Medically Necessary

Covered when ALL of the following are met

Aquablation medical necessity: The individual is age 45 or older; has persistent or progressive lower urinary tract symptoms despite medical therapy (α1-adrenergic antagonists maximally titrated, 5α-reductase inhibitors, or combination medication therapy maximally titrated) over a trial period of no less than 6 months OR the individual is not a suitable candidate for anesthesia or is unable to tolerate medical therapy; AND IPSS score ≥12; AND prostate gland volume is between 30 and 150 cc; AND individual does not have an active urinary tract infection or prostatitis within the past year; AND individual does not have a diagnosis of urethral stricture, meatal stenosis, or bladder neck contracture; AND individual does not have a known allergy to nickel, titanium or stainless steel; AND individual has had appropriate testing to exclude diagnosis of prostate cancer.age ≥45; IPSS ≥12; prostate volume 30–150 cc

Laser-Based Procedures — Medically Necessary

Covered when ALL of the following are met

Laser procedures medical necessity: The individual is age 40 or over; has persistent or progressive lower urinary tract symptoms despite medical therapy (α1-adrenergic antagonists maximally titrated, 5α-reductase inhibitors, or combination medication therapy maximally titrated) over a trial period of no less than 6 months OR the individual is not a suitable candidate for anesthesia or is unable to tolerate medical therapy; AND IPSS score ≥12; AND individual does not have an active urinary tract infection or prostatitis within the past year; AND individual has had appropriate testing to exclude diagnosis of prostate cancer.age ≥40; IPSS ≥12

Procedures include photoselective vaporization of the prostate (PVP), holmium laser ablation (HoLAP), and holmium laser enucleation of the prostate (HoLEP) as alternatives to TURP; evidence sufficient for health outcomes.

Transurethral Incision of the Prostate (TUIP) — Medically Necessary

Covered when ALL of the following are met

TUIP medical necessity: The individual has persistent or progressive lower urinary tract symptoms despite medical therapy (α1-adrenergic antagonists maximally titrated, 5α-reductase inhibitors, or combination medication therapy maximally titrated) over a trial period of no less than 6 months OR the individual is not a suitable candidate for anesthesia or is unable to tolerate medical therapy; AND IPSS score ≥12; AND prostate anatomy demonstrates normal bladder neck without an obstructive median lobe; AND prostate size ≤30 cc; AND individual does not have an active urinary tract infection or prostatitis within the past year; AND individual has had appropriate testing to exclude diagnosis of prostate cancer.prostate size ≤30 cc; IPSS ≥12

Transurethral Microwave Therapy (TUMT) — Medically Necessary in specific circumstances

Covered when ALL of the following are met

TUMT medical necessity: The individual has persistent or progressive lower urinary tract symptoms despite medical therapy (α1-adrenergic antagonists maximally titrated, 5α-reductase inhibitors, or combination medication therapy maximally titrated) over a trial period of no less than 6 months OR is unable to tolerate medical therapy; AND IPSS score ≥12; AND individual does not have an active urinary tract infection or prostatitis within the past year; AND individual has had appropriate testing to exclude diagnosis of prostate cancer; AND the individual is not a suitable candidate for anesthesia; AND individual is not a suitable candidate for any of the procedures listed above.IPSS ≥12

Trials included men with prostate volume up to 80 cm3 and excluded median lobe obstruction.

TUIP has demonstrated significant clinical benefit versus TURP in randomized trials for small prostates (<30 g) with faster recovery and lower complication rates; evidence sufficient for health outcomes.

Laser therapies (PVP, HoLEP, HoLAP): Multiple RCTs and meta-analyses show laser procedures provide symptom reduction comparable to TURP with shorter recovery and fewer bleeding complications; evidence sufficient for health outcomes.

TUMT is an effective minimally invasive option for select patients (smaller/medium prostates or poor anesthesia candidates) with lower perioperative bleeding but higher retreatment rates; evidence sufficient for health outcomes in specific scenarios.

iTind (temporarily implanted nitinol device): iTind trials (RCTs and single-arm studies) show mixed results; network meta-analysis suggested worse short-term symptom scores versus TURP; evidence is insufficient to determine improvement in net health outcome.
