Coverage Criteria for Hematopoietic Cell Transplantation in CML

Medically Necessary Allogeneic HCT

Covered when ALL of the following are met

Allogeneic HCT (myeloablative): Myeloablative conditioning allogeneic HCT may be considered medically necessary as a treatment of chronic myeloid leukemia.

Typically for younger patients (<60 years) without significant comorbidities; see clinical judgment in document.

Allogeneic HCT (reduced-intensity): Reduced-intensity conditioning allogeneic HCT may be considered medically necessary for individuals who meet clinical criteria for an allogeneic HCT but are not candidates for myeloablative conditioning (eg, older age, organ dysfunction, poor performance status).

Includes patients typically >60 years or with significant comorbidities; intended to balance relapse risk and non-relapse mortality.

Autologous HCT is considered investigational for chronic myeloid leukemia.

Autologous hematopoietic cell transplantation (autologous HCT) for chronic myeloid leukemia (CML) is considered investigational and is not medically necessary. The policy’s evidence review found insufficient data from controlled studies to demonstrate that autologous HCT improves net health outcomes for patients with CML; case series and uncontrolled data do not support routine use for this indication.

Candidate Criteria for Allogeneic HCT

Candidates for allo-HCT include

Indications for Allo-HCT: Patients with CML who have failed or are intolerant to tyrosine kinase inhibitors, have TKI-resistant mutations (eg, T315I), or are in accelerated or blast phase where allo-HCT is the only potentially curative option.

TKI pretreatment does not worsen outcomes if HCT is needed.

Contraindications to Transplantation

Patients who are not medically fit to tolerate a myeloablative conditioning regimen (for example, those with significant organ dysfunction, advanced age, generalized debilitation, or low performance status) are considered poor candidates for myeloablative allogeneic HCT. Such comorbidities should be documented, and these patients may instead be considered for reduced-intensity conditioning (RIC) allogeneic HCT when clinically appropriate.

Background on Chronic Myeloid Leukemia

Chronic myeloid leukemia (CML) is a hematopoietic stem cell disorder driven by the BCR-ABL fusion (Philadelphia chromosome), which produces constitutive tyrosine kinase activity and results in uncontrolled myeloproliferation. The disease typically progresses from a chronic phase to accelerated and then blast phase if not controlled. The introduction of tyrosine kinase inhibitors (TKIs) has transformed CML management: TKIs are now the first-line therapy for chronic phase disease and have greatly reduced the role of hematopoietic cell transplantation (HCT). Nevertheless, HCT — particularly allogeneic HCT — remains the only potentially curative option for patients who are intolerant of, resistant to, or fail TKIs, and for those in accelerated or blast phases. Evidence suggests that prior TKI therapy does not worsen outcomes when HCT is subsequently indicated.
