Coverage Criteria for Inhaled Nitric Oxide (iNO)

Medically Necessary

Covered when ALL of the following are met:

Neonatal term/late preterm hypoxic respiratory failure: Patient is a neonate born at 34 or more weeks of gestation with hypoxic respiratory failure.gestational age >=34 weeks

Use of inhaled nitric oxide (INO) in this population is supported by randomized controlled trials and meta-analyses showing reduced need for extracorporeal membrane oxygenation (ECMO) and improvement in combined ECMO-or-death outcomes.

Investigational / Not Medically Necessary

INO is considered investigational (not supported) for the following indications:

Premature neonates born at less than or equal to 34 weeks of gestation with hypoxic respiratory failure.gestational age <=34 weeks

Multiple RCTs and meta-analyses have not shown improvements in survival or other key outcomes (eg, bronchopulmonary dysplasia); evidence is insufficient to demonstrate net health benefit.

Acute hypoxemic respiratory failure in adults/children: Adults or children with acute hypoxemic respiratory failure (including ARDS/COVID-19 related) receiving INO.none

RCTs and meta-analyses have not demonstrated mortality benefit or shorter duration of mechanical ventilation; some analyses suggest possible increased risk of renal impairment; evidence insufficient.

Postoperative congenital heart disease: Use of INO postoperatively in adults and children with congenital heart disease to manage pulmonary hypertension.none

Small RCTs and systematic reviews did not find significant benefit for postoperative management; evidence insufficient.

Lung transplantation reperfusion injury: Use of INO during and/or after graft reperfusion in lung transplantation.none

Small RCTs and systematic review concluded evidence does not support routine use after lung transplant; evidence insufficient.

Inhaled nitric oxide (INO) is not supported for routine use after lung transplantation. Multiple small randomized controlled trials and a systematic review found no statistically significant improvements in clinical outcomes with INO during or after graft reperfusion, and reviewers concluded the available evidence does not support routine use in this setting.

INO is also not supported for routine postoperative management in patients with congenital heart disease. Evidence from several small RCTs and systematic reviews did not demonstrate meaningful benefits in mortality, length of hospitalization, or other key outcomes for children post-cardiac surgery; data in adults are limited and similarly do not show improved postoperative outcomes.

Use of INO is considered investigational / not medically necessary for the following additional populations due to insufficient evidence of net health benefit:

• Premature neonates born ≤34 weeks gestation: Multiple RCTs and meta-analyses have not shown improvements in survival or major outcomes (eg, bronchopulmonary dysplasia or intracranial hemorrhage) with INO in this population.

• Adults and children with acute hypoxemic respiratory failure (including ARDS/COVID-19–related causes): Large RCTs and meta-analyses have not demonstrated mortality benefit or shorter duration of mechanical ventilation, and some analyses suggest a possible increased risk of renal impairment.

• Postoperative settings: As noted above, postoperative use in adults and children with congenital heart disease and routine use around lung transplantation are considered investigational based on available trials and systematic reviews that did not show consistent clinical benefit.

Therapy Sequencing and Clinical Context

Inhaled nitric oxide is described as part of the treatment sequence for neonatal hypoxic respiratory failure and other causes of pulmonary hypertension after standard measures have been attempted. Typical treatments to be attempted prior to iNO include oxygen supplementation, mechanical ventilation, correction of acidosis/induction of alkalosis, neuromuscular blockade or sedation, and consideration of extracorporeal membrane oxygenation (ECMO) when other therapies fail. Use of iNO may also be considered in post‑operative pulmonary hypertension (for example after congenital heart surgery) or to reduce reperfusion injury in lung transplantation when clinically appropriate.

Background

Neonatal hypoxic respiratory failure is a syndrome in which inadequate oxygenation arises from causes such as respiratory distress syndrome, persistent pulmonary hypertension of the newborn, meconium aspiration, pneumonia, or sepsis. Standard supportive care includes supplemental oxygen and mechanical ventilation, with adjunctive measures such as induction of alkalosis, neuromuscular blockade, and sedation as indicated.

Inhaled nitric oxide is a selective pulmonary vasodilator used in term and late preterm neonates to improve oxygenation and, based on randomized trials and meta-analyses, to reduce the need for extracorporeal membrane oxygenation (ECMO) or the combined outcome of ECMO or death. ECMO is an invasive rescue therapy considered when conventional and adjunctive measures fail to maintain adequate oxygenation.
