Coverage Criteria for Routine Foot Care and Nail Debridement

Medically Necessary Criteria

Covered when ALL of the following apply depending on the scenario:

Systemic disease present: Patient has a systemic disease (metabolic, neurologic, or peripheral vascular) of sufficient severity that performance of routine foot care by a nonprofessional would put the patient at risk (for example, severe circulatory impairment or areas of desensitization).

Mycotic (onychomycosis) nail debridement: Clinical evidence of mycosis of the toenail AND: for ambulatory patients, marked limitation of ambulation, pain, or secondary infection resulting from thickening/dystrophy of the infected toenail plate; for non‑ambulatory patients, pain or secondary infection resulting from thickening/dystrophy of the infected toenail plate. Additionally, definitive antifungal treatment options must have been reviewed and discussed with the patient and the attending physician must document that criteria are met.

Onychogryphosis and symptomatic nail pathology: Onychogryphosis (long‑standing thickened, hooked 'ram's horn' nail) causing marked limitation of ambulation, pain, and/or secondary infection with symptomatic indentation or minor laceration of the distal toe; or other toenail procedures when these symptomatic conditions are present.

Documented Class findings (severe peripheral involvement): Presence of one or more of the documented Class findings: Class A — non‑traumatic amputation of the foot or integral skeletal portion; Class B — advanced trophic changes (three required) such as absent posterior tibial or dorsalis pedis pulse, decreased/absent hair growth, pigmentary changes, nail thickening, thin/shiny skin; Class C — claudication, temperature changes, paresthesias, edema, burning. These findings must be documented in the medical record.

Services that are components of routine foot care are generally considered not medically necessary when performed in the absence of localized illness, injury, or symptoms involving the foot. Examples include clipping, trimming, or debridement of nails (including mycotic nails), cutting or removal of corns and calluses, shaving/paring of plantar warts when non‑definitive or palliative, and other hygienic or preventive maintenance care such as cleaning/soaking feet and application of skin creams. These exclusions apply regardless of the provider performing the service.
