Medical Necessity Criteria for Liposuction/Lipectomy for Limb Lipedema

Covered when ALL of the following are met:

Diagnostic criteria: Diagnosis of lipedema in the limbs demonstrated by: bilateral, symmetrical, disproportionate fatty tissue hypertrophy (with photographs); pain or hypersensitivity to touch in affected areas; evidence of cuff phenomenon (sparing of feet when lower extremities affected or hands when upper extremities affected); absence of pitting edema (unless comorbid lymphedema); negative Stemmer sign; documented significant physical functional impairment (e.g., difficulty ambulating or performing activities of daily living, recurrent cellulitis or skin ulcerations)

Based on clinical documentation and photographs

Conservative therapy failure: Refractory to at least 6 months of conservative medical management such as medical-grade compression garments recommended by a compression specialist, complex decongestive lymphatic therapy/manual lymph drainage, or intermittent sequential pneumatic compression>= 6 months

Conservative measures and duration must be documented in the medical record

Obesity and weight-loss requirement: When class II or class III obesity is present, failure of limb adipose hypertrophy to respond to recommended bariatric surgery or other medically supervised weight loss measures is requiredapply if BMI class II/III
