Outpatient Pain Management - Presumptive Testing

Presumptive (immunoassay) urine drug testing may be considered MEDICALLY NECESSARY when ALL of the following are met:

Outpatient pain management presumptive testing: An adequate clinical assessment of individual history and risk of substance use is performed; clinicians have knowledge of test interpretation; there is a plan for how to use test findings clinically; drug testing is ordered by a clinician during an office visit; used as baseline before initiating treatment or at time treatment is initiated, and for subsequent monitoring at a frequency appropriate to risk level (eg ORT-based schedule).

Presumptive testing should be the initial screen.

Outpatient SUD Treatment - Presumptive Testing

Presumptive (immunoassay) urine drug testing may be considered MEDICALLY NECESSARY when ALL of the following are met:

Outpatient SUD presumptive testing: Baseline screening at program entry (one time) with adequate clinical assessment, clinician knowledge of interpretation, plan for using findings, and ordered by clinician during office visit; stabilization and maintenance phase testing when an appropriate test and frequency are used for the individual's risk and substance, with documentation of rationale for tests, individual's substance use history, and how results will guide decisions.

Definitive (Confirmatory) Testing

Definitive (confirmatory) urine drug testing may be considered MEDICALLY NECESSARY when ANY of the following apply:

Definitive testing indications: When immunoassays for the relevant drug(s) are not commercially available; need to detect a specific substance not adequately identified by presumptive methods; unexpected positive or negative presumptive test results requiring confirmation; need for quantitative levels for clinical decision-making (eg treatment transition or medication changes).

Definitive testing is reserved for specific clinical situations and is performed in a laboratory; certain settings (ER, ambulatory surgery, inpatient, methadone maintenance, acute change in mental status, pregnancy exposure) do not count toward the annual 20-test limit.

Routine presumptive or definitive drug testing performed as a standing order (for example, testing at every visit without individualized assessment of risk or clinical necessity) is considered investigational and not covered. The policy requires that testing be ordered by a clinician during an office visit and that presumptive (immunoassay) testing be used as the initial screen; standing orders or universal testing without consideration of individual risk factors or whether definitive testing is required are not supported.

In outpatient pain management and outpatient substance use disorder treatment, hair drug testing and oral fluid drug testing are considered investigational and therefore not supported by this policy. This exclusion was noted in prior policy history and remains stated in the current guidance.

Any drug tests ordered by third parties (for example, supportive housing, courts, employers, or schools) are considered not medically necessary. The policy specifies that urine drug testing should be ordered by a clinician during an office visit; third-party-initiated testing does not meet that requirement and may be denied.

Historical policy language (noted in prior history entries) treated hair and oral fluid testing in outpatient pain management and substance abuse treatment as investigational. The policy history documents these prior statements and clarifications while the current policy text continues to identify hair and oral fluid matrices as investigational in outpatient settings.

Urine drug testing (presumptive or definitive) is considered not medically necessary when performed in excess of 20 services per member per 365 days. The policy also clarifies that certain settings and scenarios (for example, emergency rooms, ambulatory surgery, inpatient services, methadone maintenance, abrupt change in mental status, and pregnancy-related testing) may be considered medically necessary and do not count toward this limit.

Prior policy history indicated that services not meeting medical necessity criteria were labeled as 'investigational' (the terminology and phrasing have evolved over time). For services described in this policy, providers must follow coding/billing rules, obtain informed consent, document the testing plan and results, and follow the required testing sequence and clinical approach described below.

Coding and billing: Bill using the HCPCS/CPT codes appropriate to the test performed (presumptive immunoassay vs definitive/confirmatory testing). Clinics should use the HCPCS/CPT codes listed in the policy coding section and bill using those specific codes. Blue Cross Blue Shield of Massachusetts may authorize up to 20 presumptive or definitive urine drug test services per member per 365 days; tests performed in inpatient, emergency room, ambulatory surgery settings, methadone maintenance, pregnancy-related exposures, and abrupt change in mental status do not count toward this limit.
Informed consent requirement: Full informed consent must be obtained and documented before urine drug testing. Patients must be informed about the specific drug testing protocol to be used, how results will be used, and must provide written agreement to the monitoring plan. A patient’s refusal to consent should be considered in assessment of ability to adhere to treatment.
Documentation and consent: The medical record must document an adequate clinical assessment of substance use history and individual risk, clinicians’ knowledge of test interpretation, the rationale for specific tests ordered, and how results will guide clinical decision-making. The record should also note the informed consent discussion and the agreed-upon testing protocol.
