Background & Evidence

Background: Intra-articular hyaluronan (viscosupplementation) is proposed to restore the viscoelastic properties of synovial fluid to improve pain and function in osteoarthritis. The evidence base for knee osteoarthritis includes many randomized controlled trials and meta-analyses but shows mixed results with generally small treatment effects and concerns for bias, such that the literature does not demonstrate a definitive, clinically meaningful net health benefit. For joints other than the knee (eg, hip, shoulder, ankle), randomized trials and systematic reviews have not provided sufficient evidence of clinically important benefit. Based on the Blue Cross Blue Shield Association policy, intra-articular hyaluronan injections of the knee are considered not medically necessary and injections for all other joints are considered investigational.
