Member is a commercial member (Managed Care HMO/POS, PPO, or Indemnity).
Device is a limb compression device intended for postsurgical venous thromboembolism (VTE) prophylaxis for home use.
The individual has a contraindication to pharmacologic anticoagulation (e.g., high risk of bleeding) that precludes use of pharmacologic VTE prophylaxis.
- ANY of the following
  - After major orthopedic surgery (total hip arthroplasty, total knee arthroplasty, or hip fracture surgery).
  - After major non-orthopedic surgery or other orthopedic procedures in individuals who are at moderate or high risk of VTE.
