Surgical deactivation targets presumed trigger sites (frontal, temporal, rhinogenic, occipital) based on the theory that irritation or inflammation of trigeminal nerve branches by surrounding musculature, bony foramina, fascia bands, or vessels produces headaches. Procedures vary by site and may involve removal or decompression of nerve-associated tissues (eg, resection of glabellar muscles, removal of a segment of the zygomatico-temporal branch, septoplasty/turbinectomy for rhinogenic triggers, or removal of semispinalis capitis muscle segments and placement of a subcutaneous flap to prevent occipital nerve impingement).

Randomized controlled trials: Three RCTs reported better 12-month outcomes with decompression surgery versus control; however, the trials had methodological limitations (eg, unclear/variable patient selection and procedural variability) and only one RCT used a sham control with patient blinding.
