Covered when ALL of the following are met

Eligibility criteria for responsive neurostimulation: Member is 18 years or older; diagnosis of focal seizures with 1 or 2 well-localized seizure foci identified; average of 3 or more disabling seizures per month over the prior 3 months (eg, motor focal, complex focal, or secondary generalized seizures); refractory to medical therapy (failed 2 or more appropriate antiepileptic medications at therapeutic doses); not a candidate for focal resective epilepsy surgery (eg, epileptic focus near eloquent cerebral cortex or bilateral temporal epilepsy); no contraindications to RNS placement (eg, 3 or more specific seizure foci, primary generalized epilepsy, or rapidly progressive neurologic disorder).

These criteria apply to commercial and Medicare members as specified in policy.

Responsive neurostimulation (eg, NeuroPace RNS) is considered investigational for all indications not explicitly meeting the medical necessity criteria listed in this policy. Coverage is limited to individuals who meet ALL specified criteria (age ≥18 years, 1–2 well-localized focal seizure foci, average of ≥3 disabling seizures per month over the prior 3 months, failure of ≥2 appropriate antiepileptic medications at therapeutic doses, not a candidate for focal resective surgery, and no contraindications to RNS placement).

Contraindications to responsive neurostimulation device placement include: having 3 or more specific seizure foci, the presence of primary generalized epilepsy, or a rapidly progressive neurologic disorder. Individuals with any of these conditions are not eligible for RNS placement under this policy.

Preauthorization is required. For services described in this policy, precertification/preauthorization IS REQUIRED if the procedure is performed inpatient. For outpatient settings, the procedure is treated as inpatient for purposes of prior authorization under Commercial Managed Care (HMO and POS), Commercial PPO and Indemnity, Medicare HMO Blue SM, and Medicare PPO Blue SM — prior authorization may therefore be required.

Preauthorization required for inpatient procedures.
Outpatient procedures are considered inpatient for prior authorization across listed product lines.

Background and Clinical Context

Responsive neurostimulation systems (eg, the NeuroPace RNS) are implanted devices that continuously monitor brain electrical activity via electrocorticography and deliver targeted electrical stimulation to cortical and/or deep electrodes when epileptiform patterns are detected. The system’s aim is to interrupt or abort evolving seizures in individuals with well-localized focal epilepsy who have not responded to medical therapy and who are not candidates for resective surgery.

Medication Failure Requirement

Members must have failed adequate medical therapy prior to RNS consideration. Specifically, responsive neurostimulation is considered medically necessary only for individuals who are refractory to medical therapy, defined as failure of 2 or more appropriate antiepileptic medications at therapeutic doses.

Failure of >= 2 appropriate antiepileptic medications at therapeutic doses is required.
Document medication names, doses, treatment durations, and response.
