Background and Evidence

Insulin potentiation therapy (IPT) uses insulin administered with chemotherapy to potentiate anticancer drug effects and allow reduced chemotherapy dosing. Proposed mechanisms include selective increased tumor sensitivity related to overexpression of certain insulin-like growth factor (IGF) receptors and increased cell membrane permeability that may raise intracellular concentrations of cytotoxic drugs.

The evidence base is limited: much of the information comes from short-term anecdotal reports, with a single randomized controlled trial suggesting possible effect on tumor progression at 8 weeks but with no survival or longer-term data. Overall, effectiveness has not been established in pharmacokinetic, survival, or long-term clinical outcome studies.
