Policy stance: Devices using bioimpedance (bioelectrical impedance spectroscopy) are considered INVESTIGATIONAL for use in the diagnosis, surveillance, or treatment of individuals with lymphedema, including use in subclinical secondary lymphedema.

Applies to Commercial Members (Managed Care HMO and POS), PPO, Indemnity, Medicare HMO BlueSM, and Medicare PPO BlueSM; outpatient use is not a covered service and inpatient procedures require precertification as specified in the policy.

Evidence summary: The evidence includes systematic reviews, one randomized controlled trial (PREVENT RCT), one prospective comparative observational study, and multiple uncontrolled observational studies. Diagnostic accuracy studies have shown poor correlation between bioimpedance and reference standards (volume displacement or circumferential measurement). In the PREVENT RCT BIS triggered intervention less often than tape measurement (20.1% vs 27.5%) and fewer BIS patients progressed (7.9% vs 19.2%), but the trial was limited by open-label design and lack of important health outcomes reporting. The prospective comparative study and retrospective series have methodological limitations. Overall, the evidence is insufficient to demonstrate an improvement in net health outcome.

These limitations support the investigational determination.

Outpatient use of bioimpedance spectroscopy (BIS) devices for diagnosis, surveillance, or treatment of lymphedema is not a covered service for the following product lines: Commercial PPO and Indemnity, Medicare HMO BlueSM, and Medicare PPO BlueSM. Precertification/preauthorization is required when the procedure is performed inpatient; outpatient services described in this policy are excluded from coverage for the product lines listed above.

Clinical Background

Lymphedema is an accumulation of fluid due to disruption of lymphatic drainage and is commonly secondary to cancer treatment (for example, breast cancer). It typically presents as nonpitting swelling of an extremity associated with changes in skin and subcutaneous tissue and may cause pain, decreased function, and recurrent infection.

Staging of lymphedema ranges from Stage 0 (latent or subclinical), in which swelling is not yet evident despite impaired lymph transport and patients may report subtle symptoms, through clinical stages up to Stage III (severe). Management depends on stage and may include elevation, compression, exercise, manual lymphatic drainage, complete decongestive therapy, pneumatic compression, and, for advanced disease, surgical options.

Bioimpedance spectroscopy (BIS) measures the opposition to an applied electrical current (impedance) through tissue as an indirect estimate of extracellular fluid volume and has been proposed for early detection and surveillance of subclinical lymphedema.
