Background, Evidence, and Definitions

Background: Fecal calprotectin is a stable fecal protein marker of neutrophilic intestinal inflammation used to distinguish IBD from non-IBD, to help select patients for endoscopy, to monitor disease activity, and to predict relapse. A commonly used cutoff is 50 µg/g (recommended lower bound for adults and children >4 years), but values vary by age (higher in neonates/infants/young children and moderately increased up to 100 µg/g in individuals >65 years) and may be elevated by medications (eg, NSAIDs, proton pump inhibitors) and other clinical situations (eg, GI bleeding).
