After skeletal maturation, bone remodeling continues through coupled processes of osteoclast‑mediated resorption and osteoblast‑mediated formation. Biochemical bone turnover markers—measurable in serum or urine—reflect formation or resorption activity and have been proposed as adjuncts to bone mineral density testing to assess fracture risk, monitor osteoporosis therapy, or gauge activity in high bone‑turnover conditions. However, current evidence does not support that routine measurement of these markers improves clinical outcomes or alters management in a way that reduces fracture risk; therefore such testing is designated investigational in the scenarios described in this policy.
