Coverage Criteria for Bone Mineral Density Testing

Initial or Screening DXA

Covered when ANY of the following are met for initial central DXA:

Initial screening: Women age 65 and olderage >=65

Initial screening: Men age 70 and olderage >=70

Initial screening: Younger postmenopausal women with an elevated risk factor assessmentelevated risk by assessment

Noncentral modalities

Alternative modalities (peripheral DXA, QCT, ultrasound): Use of ultrasound densitometry, quantitative CT, or peripheral DXA for screening or monitoring is not supported by evidence to select patients who will benefit from treatment

These technologies are not commonly used in practice and evidence is insufficient

Ultrasound densitometry, dual x-ray absorptiometry of peripheral sites, and quantitative computed tomography are considered investigational for routine BMD measurement. Peripheral DXA may be medically necessary only when central (hip/spine) DXA is not feasible or when a forearm (radius) measurement is essential for managing hyperparathyroidism. Refer to CPT codes 77080 and 77081 for central and peripheral DXA descriptions; investigational codes include 0691T, 0815T, and 76977.

Use of noncentral modalities (peripheral DXA, quantitative CT, ultrasound) to select patients for osteoporosis treatment is not supported by current evidence. Systematic reviews and observational studies have not demonstrated that these alternative techniques identify patients who will benefit from pharmacologic therapy, and no evidence shows that repeat measurements with these modalities improve net health outcomes.

Initial or repeat BMD measurement should be performed only when the results will influence clinical management. Central DXA (hip or spine) is the reference standard for assessing fracture risk and guiding pharmacologic therapy; testing is indicated when an individual meets established screening criteria (for example, women age 65+ or men age 70+, younger adults with elevated risk, or those with conditions or medications associated with bone loss).

Repeat measurement with noncentral technologies (peripheral DXA, quantitative CT, ultrasound) lacks evidence of clinical utility for monitoring and therefore is not supported. There is little to no evidence that repeat assessments using these modalities improve fracture-related outcomes or meaningfully inform treatment decisions.

Background and Rationale

Osteoporosis is common in older adults due to age- and menopause-related bone loss; fracture risk is assessed primarily using bone mineral density (BMD) measured by dual x-ray absorptiometry (DXA). Central DXA of the hip or spine is the reference standard for risk prediction because it identifies individuals at increased fracture risk and can guide pharmacologic therapy. Evidence for repeat testing with central DXA is mixed: guidelines support risk-based intervals (e.g., 3–5 years for low-risk screening, 1–2 years for high-risk or osteopenia near treatment threshold, and 1–3 years when monitoring pharmacologic therapy), but optimal frequency is not well established and within-person measurement variability may exceed treatment-related changes.
