Background and evidence summary: Numerous lipid and non-lipid biomarkers (including apolipoprotein B, apolipoprotein AI, apolipoprotein E, HDL and LDL subclasses, lipoprotein(a), B-type natriuretic peptide, cystatin C, fibrinogen, and leptin) and commercially available multi-marker CVD panels have been proposed to improve cardiovascular risk prediction beyond traditional risk factors and simple lipid measures.

Clinical utility conclusions: Cohort studies, meta-analyses, and systematic reviews have shown associations between many of these markers and cardiovascular risk and have identified some incremental predictive value (notably for apoB and apoAI). However, there is no high-quality evidence that measuring these nontraditional biomarkers or using multi-marker panels leads to management changes that improve clinical outcomes compared with traditional lipid measures; therefore the evidence is insufficient to demonstrate net health benefit.
