Medically Necessary Indications

Transcatheter RFA, cryoablation or pulsed field ablation is MEDICALLY NECESSARY when ALL of the following apply:

Procedure type: Procedure is transcatheter radiofrequency ablation (RFA), cryoablation, or pulsed field ablation to treat atrial fibrillation (may include pulmonary vein isolation and/or focal ablation).
See policy: pulsed field ablation added as medically necessary per 8/1/2025.
AF type and coding: Member has symptomatic paroxysmal or symptomatic persistent atrial fibrillation.
Relevant ICD-10 codes include I48.0, I48.11, I48.19, I48.20, I48.21, I48.91 (paroxysmal/persistent/permanent and unspecified AF) and atrial flutter codes I48.3, I48.4, I48.92; submit appropriate ICD-10 diagnosis code(s) with CPT codes 93656/93657.

Background

Catheter-based ablation techniques target arrhythmogenic foci, most commonly near the pulmonary veins, using percutaneous catheters. Traditional approaches include radiofrequency ablation (RFA) with circumferential pulmonary vein isolation and cryoablation (eg, cryoballoon) that can allow a one-shot anatomic approach. Pulsed field ablation (PFA) is a newer technique that delivers brief electrical pulses and has been recently approved in the US. Ablation is used for rhythm control and may be offered as an alternative to atrioventricular nodal ablation with pacemaker implantation in selected patients with heart failure. Repeat ablation procedures are commonly performed for recurrent AF or new atrial flutter and are generally more limited in scope than the initial procedure.

Evidence from randomized trials and systematic reviews supports that RFA and cryoablation are effective for drug-refractory symptomatic paroxysmal and persistent atrial fibrillation and improve net health outcomes. Randomized controlled trials of PFA have demonstrated noninferiority to thermal ablation for short-term effectiveness and safety, but evidence is insufficient to determine whether PFA improves long-term net health outcomes.

Initial Therapy (first-line) - Selected Cases

Covered as initial treatment when ALL of the following are met:

Recurrent symptomatic paroxysmal AF: Individual has recurrent symptomatic paroxysmal atrial fibrillation.>1 episode, with 4 or fewer episodes in the previous 6 months
Threshold definition from policy: eligibility for first-line ablation when rhythm-control strategy desired.
Treatment strategy desired: A rhythm-control strategy is desired by the clinician and/or patient.
First-line ablation permitted only for selected recurrent paroxysmal AF per threshold above.

Repeat Procedures

Up to 3 repeat transcatheter ablations may be MEDICALLY NECESSARY when the following are met:

Recurrence or flutter: Recurrence of atrial fibrillation and/or development of atrial flutter following the initial procedure.
Clinical rationale supports repeat procedure (scope generally more limited than initial procedure).
Policy limit: Up to 3 repeat transcatheter ablations may be MEDICALLY NECESSARY.

Investigational / Not Medically Necessary

Transcatheter RFA, cryoablation, and pulsed field ablation is considered INVESTIGATIONAL when ANY of the following apply:

Does not meet medically necessary criteria: Atrial fibrillation case does not meet the specific criteria outlined in the medically necessary sections above.
Such cases are considered investigational / not medically necessary per policy.
